The Opportunity:

The Nurse Practitioner will work collaboratively on the delivery of resident care to meet individual needs within his/her level of competency. This role provides quality and safe resident care which helps to support and align resident needs.

What Is In It For You:

1. Collaborate and liaise with the MD, DOC and interdisciplinary team in care delivery.
2. Assess, diagnose, and evaluate acute and chronic illness and other changes in resident’s conditions, prescribe treatments, diagnostics and follow up care.
3. Support and advocate for residents’ best interests while engaging with the resident / family / substitute decision maker (SDM) regarding care.
4. Participate in resident care conferences.
5. Provide clinical leadership to the LTC team members centered around the resident care needs which can include resident move in/out, hospital discharge planning, palliative and end-of-life care, pain management, dementia care and responsive behavior management.
6. Participate in identifying, analyzing, and interpreting trends in resident care and professional nursing practice to determine priorities for educational programming for team members, residents, and families.

What You Will Need:

1. Minimum one year clinical and/or working experience in geriatric, rehabilitation, or long-term care nursing.
2. Some leadership knowledge/experience is considered an asset.
3. Certifications obtained or in progress that include gerontology, mental health, behavioral support, and palliative care are an asset.
4. Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice.
5. Working knowledge of the Long Term Care Act, Program Standards for Long Term Care, Health Disciplines Act, Occupational Health & Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada) and Coroners Act, is considered an asset.
