The Nurse Practitioner [NP(EC)] for the Nurse Led Outreach Team (NLOT) program uses advanced practice clinical skills to provide direct clinical care, education, research, consultation and treatment in the area of Geriatrics to support the acute care needs of residents in LTC, Retirement Homes or other Seniors Congregate settings within the Orillia Soldiers’ Memorial Hospital (OSMH), Couchiching Ontario Health Team (COHT) catchment areas and Muskoka as required. This position will include travelling to all areas serviced within the North Simcoe Muskoka region.
Education:
• BScN required
• Nurse Practitioner Certification required
• Masters degree in Nursing/Nurse Practitioner Stream is preferred
• Acute Care Nurse Practitioner Certification preferred
• Gerontology Nursing Certification is preferred
• BCLS is required
Experience:
• Minimum three (3) years experience in advanced practice nursing in geriatrics/seniors care preferred
• Minimum three (3) years relevant clinical specialty-related experience as a Registered Nurse required
• Recent experience in a leadership role within an inter-professional team preferred
• Relevant knowledge of LTC environment, hospital and community resources preferred
