Job Overview -

As a Nurse Case Manager at SRx Health Solutions your primary responsibility would be to coordinate the care and treatment of patients with complex medical conditions who require specialty medication. You would work closely with patients, physicians, pharmacists, and other healthcare professionals to ensure that patients receive high-quality care that is tailored to their individual needs.

What you will be doing, including, but not limited to:

Clinical Skills

  • Proficient in conducting patient assessments, including physical examinations and medical history reviews.
  • Skilled in performing medical procedures relevant to the specialty.
  • Ability to organize and interpret diagnostic tests and results accurately.
Communication Skills
  • Ability to communicate effectively with patients, families, and interdisciplinary team members.
  • Empathetic and compassionate approach to patient care.
  • Consistent communication with the Nursing Manager and Site Supervisor.
Critical Thinking And Problem-Solving
  • Demonstrates critical thinking skills in assessing and managing complex patient situations.
  • Ability to make quick and accurate decisions in urgent situations.
  • Effective critical thinking skills to address challenges in patient care.
Teamwork And Collaboration
  • Works well as part of an interdisciplinary team, including nurses, physicians, pharmacists, etc.
  • Collaborate effectively with other healthcare professionals to ensure coordinated patient care.
  • Open to receiving feedback and implementing changes based on team input.

  • Demonstrates a commitment to providing patient-centered care.
  • Respects patient preferences, values, and cultural backgrounds.
  • Engage patients in shared decision-making and treatment planning.
Documentation And Record-Keeping
  • Maintains accurate and thorough documentation of patient assessments, interventions, and outcomes.
  • Familiarity with electronic medical record (EMR) systems and documentation requirements.

  • Comply with all legal and regulatory requirements related to healthcare delivery.

Job Specifications & Qualifications
  • Proof of valid nursing or medical license in the appropriate jurisdiction with current Nursing Registration.
  • Proof of valid professional liability insurance coverage, renewed on an annual basis.
  • Clinical Background: Previous experience in a clinical setting considered an asset.
  • Knowledge of Biologics: Familiarity with biologic medications and therapies assets.
  • Completed education from an accredited nursing school or medical program.
  • Continuous education and professional development in their field.
  • Excellent verbal and written communication skills.
  • Adheres to safety protocols and infection control measures to ensure patient and staff safety.
  • Knowledgeable about standard precautions, isolation procedures, and proper use of personal protective equipment (PPE).
  • Understands and adheres to ethical principles and professional standards of practice.
  • No. of years of exp.: 1-5 years
  • Specialty: Rheumatology
  • Travel requirements/work setting: On site (Monday- Thursday), Remote (Fridays) Full time, must own/have access to their own vehicle to commute on a daily basis.
  • Familiarity with EMR applications an asset.
