UNIT PROFILE
The Concurrent Disorders Outpatient Program offers services for individuals who are experiencing a significant impact on their functioning due to problematic and persistent mental health and addiction concerns. An integrated care model ensures a seamless integration of psychiatric and substance abuse interventions. Services are time-limited and goal-focused. The services provided will be based on an individualized assessment of a client’s needs and the majority of treatment will be delivered via group programming.

POSITION SUMMARY
The Mental Health Worker provides consults, counselling/therapy to individuals who have a concurrent disorder for outpatients, including the use of counselling modalities such as DBT and CBT. The Mental Health Worker will also facilitate group, support patients in transition from inpatient to outpatient services, and to community services. The Mental Health Worker works as part of a multi-disciplinary team and in close collaboration with members of the Concurrent Disorder Capacity Building Team and the Rapid Access Addiction Medicine (RAAM) Team.

QUALIFICATIONS
• Degree/Diploma in Nursing (RN) or Social Work (BSW or MSW) or Occupational Therapy OR current registration in good standing with the College of Psychotherapists with a Degree in a health-related discipline required
• Current registration with your college in good standing: College of Nurses (RN), Ontario College of Social Workers & Social Service Workers, College of Occupational Therapists of Ontario or College of Registered Psychotherapists of Ontario
• Completion of a certificate program in addictions/concurrent disorders from an accredited educational institution preferred
• Current First Aid/CPR certification
• CPI-Crisis Prevention Training (or successful completion as part of clinical orientation) is required
• Minimum of 5 years recent related clinical experience in concurrent disorders preferred
• Three-year recent related clinical experience in health teaching and group facilitation; both in person and virtually preferred
• Demonstrated knowledge and competence of individual counseling and group skills (i.e. DBT, MI, CBT, Behavioral Activation)
• 1-year experience in measurement-based care and program development preferred
• Demonstrated ability to complete comprehensive assessments, develop and implement client-centered integrated treatment plans in collaboration with other involved service providers within the hospital and community
• Demonstrated knowledge of the stages of change model and strong skills in the utilization of Motivational Interviewing
• Demonstrated experience working with a harm reduction model including the provision of Naloxone, anti-craving medications, and Opioid Replacement Therapy (ORT)
• Demonstrated knowledge and skills in providing formal and informal support to loved ones
• Demonstrated knowledge of and ability to access formal and informal community services and resources

Concurrent Disorders-Specific Qualifications
• Completion of a certificate program in addictions/concurrent disorders from an accredited educational institution preferred
• Completion of the Certificate Program in Intensive Tobacco Cessation Counselling (TEACH) preferred
• Demonstrated competence in the facilitation of addictions/concurrent disorder groups (relapse prevention)
• Demonstrated ability to administer addiction-specific screening tools (i.e. CAGE-AID, GAIN)
• Demonstrated ability to develop and implement integrated treatment plans (related to relapse prevention and harm reduction) in collaboration with other involved service providers within the hospital and community
• Demonstrated knowledge of and ability to access formal and informal community services and resources
• Experience in leading case formulation, case conferences, and providing clinical supervision
• Ability to provide clinical consultation through the demonstration of therapeutic techniques and interventions specific to concurrent disorders
