Position Summary
The role of the MPCN Licensed Practical Nurse is to work collaboratively within a primary care clinic offering health services to newly arrived refugees across all age groups. The nurse will work within scope of practice to participate in the provision of care to meet the needs of patients with a variety of clinical conditions. Specific duties will vary day to day.
Key Responsibilities
- Works within scope of practice as part of the clinic team to provide support to physicians and patients in day-to-day clinical activities.
- Performs a variety of nursing duties to provide consistent high-quality care to meet the needs of patients through the lifespan.
- Cooperates in developing strategies for improving the provision of health screening and follow-up care for the practice population.
- Initiates appropriate investigations, in accordance with physician protocols.
- Assists patients to navigate through the health system. Provides education and teaching as needed, assists with booking relevant appointments.
- Communicates with patients to determine the next appropriate health services required, based on presenting concerns and test results.
- Provides direction in accessing appropriate local and regional health programs.
- Ensures the maintenance of accurate records in line with practice policy.
- Other duties as assigned to maintain seamless patient flow throughout office visits.
- Assists with the development, implementation and improvement of clinic processes and programs as required. Participates in meetings with stakeholders and partners as relevant.
Qualifications
Education and Experience:
- Successful completion of an accredited Practical Nurse diploma program.
- Current CLPNA registration, CPR and BCLS.
- Successful completion of the following: Medication Administration / Intramuscular and Intradermal injection course & Immunization.
- Five (5) years or more of related nursing experience.
- Primary Care delivery to all age groups.
Knowledge, Skills and Abilities:
- Knowledge of nursing processes and practices, including intervention strategies and therapies.
- Strong, effective written and oral communication skills, high level of perceptiveness.
- Critical thinking skills.
- Ability to build good working relationships with healthcare professionals, clients and other members of the community.
- Ability to prioritize, manage time effectively and be flexible in a very active work environment.
- Respect and understanding of confidentiality.
- Advanced skills in health promotion, health screening, assessment and education.
- Experience in developing priorities with variable workloads. Strong time management skills.
- Initiative, accountability, sound decision-making skills.
- Experience working in a team setting with other health professionals.
- Basic computer skills, ability to use Electronic Medical Records.

- Clinical experience in primary care nursing, community, gerontology and/or chronic disease management.
- Experience working with diverse and vulnerable populations.
- Familiarity with using telephone-based interpretation (e.g. Language Line).
- Multilingual.
