Come work as a Community Health Nurse with Vancouver Coastal Health (VCH)! Vancouver Coastal Health is looking for a Licensed Practical Nurse to join the Home Health team as a Community Health Nurse at Pender Community Health Centre in Vancouver, B.C. Apply today to join our team!

As a Licensed Practical Nurse with VCH you will:

  • Provide direct care to stable and predictable clients experiencing chronic disease, frailty, and/or other conditions that require supports to live independently at home.
  • Use standardized assessment and screening tools, such as the Resident Assessment Instruments (RAI-HC), to assess, plan, implement and evaluate care, including home support services and accessing community resources.
  • Establish and maintain therapeutic relationships based on mutual respect and client and family-centered care.
  • Support clients and families to achieve and maintain optimal health based on client needs and goals.

ABOUT HOME HEALTH PROGRAMS:

Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.

THE CLIENT POPULATION:

Home Health clients typically have ongoing health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence.

Qualifications

Education & Experience

  • Graduation from a recognized program for Licensed Practical Nurses. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years recent experience working as a Licensed Practical Nurse providing care in acute care (medical/surgical/palliative) and one (1) year in community settings or an equivalent combination of training, education and experience. Valid BC Drivers License. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.

Knowledge, Skills & Abilities

  • Demonstrated ability in nursing practice related to designated client group including stable and predictable clients experiencing chronic disease, frailty and/or other conditions that require supports to live independently at home.
  • Demonstrated knowledge of pathophysiology of common diagnoses related to designated client group.
  • Demonstrated ability to provide nursing assessments & interventions related to common diagnoses and specific clinical situations relevant to the designated client group.
  • Demonstrated ability to anticipate potential complications and care issues relevant to the designated client group by communicating with the RN.
  • Demonstrated ability in correct usage of equipment and maintains competency for client population.
  • Demonstrated ability to document client care on appropriate systems/forms in accordance with organizational and unit standards in a timely and accurate manner, including RAI-HC reassessments, Financial Assessments, and Home Support Orders.
  • Demonstrated skill in chronic disease and palliative symptom management of clients.
  • Demonstrated skill in performing LPN scope of practice and standards of care relevant to designated client population including infection control, etc.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check. As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
