Your Opportunity:
The Licensed Practical Nurse (LPN) Interventionist – Wound Clinic works as a member of a health care team, collaborating on the delivery of care to meet the needs of clients. The LPN applies knowledge-based nursing in collaboration with other health care professionals to promote an optimal state of health for a diverse clientele in a variety of settings. The LPN in Home Care (HC) works as a member of an interprofessional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span including but not limited to clients requiring wound care. In collaboration with the Registered Nurse, the LPN is responsible to assess, plan, implement and evaluate the needs of HC clients and their caregivers/families. This position reports to the CAT Team Area Manager and Care Manager. There is also an expectation to be a mobile worker. Requires travel to care for clients in their homes or in the community. Wound Clinic and community positions require an advanced level of skin and wound management knowledge and skills. You will be required to walk and stand on your feet for approximately 75% of your shift and work in the home environments.

You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Required Qualifications:
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
Min 3 years experience with Integrated Home Care required. Current experience (within 2 years) with skin and wound care management (including IHC courses). Minimum of two years current licensed practical nursing experience. There is also an expectation to be a mobile worker. Requires travel to care for clients in their homes or in the community. You will be responsible to carry and move equipment, use and operate a laptop and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: A push/pull of up to 50lbs for compression therapy on a regular basis is an expectation of this job. Learning resources and training are provided to safely perform work.
Preferred Qualifications:
Completion of a post-basic Medication Admin Course for LPN’s, Recent work experience in a community-based setting, Home Care experience preferred. Experience in case management practice and completion of McMaster AHS Case Management, a keen interest in wound care and a willingness to acquire expertise through education and training. Ability to perform all work-related duties in accordance with regulated legislature education preferred.
