HomeTeam Medical Services has been providing unique primary home-based interdisciplinary health care for seniors in the Greater Victoria area for 20 years. We are on the cutting edge of innovative home-based care for older adults that is evidence-based, with an integrated team of doctors, nurses, physiotherapists, and rehabilitation assistants.


We are responsive, and pride ourselves on providing the best possible care for our patients and support for their family caregivers, as well as other community and hospital workers. Our goals are to improve seniors’ quality of life, maximize function, prevent transfers to hospital or moves to nursing homes and reduce caregiver stress.


Our patients are generally living higher-functioning, vibrant lives, and we pride ourselves in helping them stay that way. This job does not involve any LTC work, all of our patients are living in the community.


Applicants must be currently located in the Greater Victoria Area, and licensed to practice as an LPN in British Columbia.

Skills /Knowledge/Experience

  • Communication- excellence in and respectful verbal and written communication skills with clients, caregivers and health care professionals.
  • Computer skills- with electronic medical records, MS Office 365, MS Teams. Familiarity with Oscar EMR is an asset but not required.
  • Time-management skills; adaptable and organized; ability to work independently.
  • Knowledge of, and adherence to, medical privacy standards.
  • Knowledge of community services for seniors.
  • Experience as an LPN (minimum 1 year).
  • Experience with community-based care an asset.
  • Valid driver's license and access to reliable vehicle.

Tasks


As a practicing Licensed Practical Nurse, you will stay up to date with best practices in your field and use this knowledge to deliver best care to our patients. You will also have an opportunity to work closely with other employees at HomeTeam Medical Services to ensure a happy, healthy, and high-performing work environment.


Working under the supervision of an RN, the specific responsibilities will include, but are not limited to:


  • Provide nursing care to patients, within your scope of practice, including:
    • Detailed history taking.
    • Performing standardized assessments and monitoring.
    • Assessing patients with acute and chronic problems, including taking vital signs, and completing objective and subjective outcome measures for screening and monitoring change.
    • Collecting specimens.
    • Taking steps to control infection.
    • Monitoring and education regarding medications and their side effects.
    • Monitoring and reporting on patient nutritional status.
    • Assessing wounds and applying dressings.
    • Removing sutures.
    • Ear syringing.
    • Administering injections (Prolia, vitamin B12, Depo-Medrol, etc.).
    • Administering vaccinations (Pneumovax, tetanus, flu vaccine, Shingrix).
    • Glucometer checks and knowledge of CGM systems.
    • Use of chemstrips for urine testing.
    • COVID swab testing.
    • Screening for chronic and secondary conditions to promote prevention and early intervention.
    • Consultation that provides professional advice and solutions as part of the interprofessional team collaboration.
    • Communication with patients, patients’ families, interdisciplinary team members, and others to achieve collaboration and service coordination.

This is a community-based position. Geographic area includes Victoria, Oak Bay, Gordon Head, Royal Oak, and Esquimalt. Mileage reimbursed according to industry standards. Access to a cellphone is required; reimbursement will be provided for use of your personal cellular telephone.


Applicants must be currently located in the Greater Victoria Area, and licensed to practice as an LPN in British Columbia.


No evening or weekend work required.

Generous salary is commensurate with current industry rates and experience.

The position includes Doctors of BC benefits (prescriptions, massage, dental, vision, etc.) for full-time employees, 4 weeks of paid vacation (3 weeks for the first year of employment), monthly paid flex days, as well as paid sick days.

Must be eligible for work in British Columbia, Canada. Background check will be required.

The position is full-time, but we would consider part-time for the right applicant.

Start date flexible, November/December May 2024.
