Adelphi is seeking a Licensed Physical Therapist (PT) to provide services at a facility in Arkadelphia, Arkansas. Requirements:
Must be licensed as a Physical Therapist by the Arkansas State Board of Physical Therapy.
Minimum of seven (7) years of experience working with individuals with intellectual disabilities in a long-term care setting.
Responsibilities:
Provide direct and indirect physical therapy services per physician’s orders.
Conduct initial and annual evaluations within 14 days of referral, including interpretation, treatment recommendations, and summary reports.
Develop and maintain individualized therapy plans aligned with ICF/IID and CARF standards.
Deliver therapy in accordance with the individual’s Interdisciplinary Team (IDT) plan and schedule.
Train and supervise staff on therapy techniques and interventions.
Participate in: Initial and annual IDT meetings, Medical staff meetings (monthly), IPP sessions (Initial, 30-Day Review, and Annual) and Discharge planning.
Provide emergency on-call physical therapy services.
Submit detailed documentation, including: Monthly and annual evaluation reports, Progress notes, treatment plans, short/long-term goals, Discharge plans and IPP-compliant therapy records (ICF standard W319).
Report all incidents per DHS Policy 1090.
Comply with all federal/state regulations including HIPAA, ICF/IID, CARF, and DHS policies.
Assist in assessing equipment needs and facilitate purchases with AHDC.
