Here is what you will be doing:

The Lead System Strategy, Planning, Design and Implementation (Home Care, Community Care and Community Support Services) is responsible for leading health system planning initiatives, policy development, and Home Care, Community Care and Community Support Services sector activities in the North West region. This role contributes to building an integrated, sustainable, and patient-centred health care system. As a subject matter expert in home and community services, the Lead will ensure a strong community care focus, identifying trends based on research and data, and will work collaboratively with the Ontario Health North West team to address challenges, identify opportunities, and troubleshoot emerging issues. This position is regional in scope and will have a strong alignment to home and community services activities across the North West region.

Key Responsibilities:

  • Serve as the main point of contact for coordinating system-level work related to the Home Care, Community Care and Community Support Services in the North West.
  • Lead initiatives related to the Home Care, Community Care and Community Support Services by utilizing Ontario Health’s Annual Business Plan and advancing strategic priorities in collaboration with the North West System Strategy, Planning, Design, and Implementation Team.
  • Apply expertise in Home Care, Community Care and Community Support Services delivery to health system planning and integration efforts, including the analysis of local health system data to identify population health needs, service gaps, and opportunities for integrated service delivery in the North West region.
  • Foster regional relationships among health service providers across the sector to improve population health, enhance patient experience, and sustain integrated service delivery.
  • Provide support to encourage the movement of ALC patients from acute care to appropriate community-based care settings, in collaboration with the North West System Strategy, Planning, Design, and Implementation team.
  • Promote a ‘Home First’ approach within Home Care, Community Care and Community Support Services, emphasizing intersectoral collaboration and efficient discharge planning.
  • Monitor Home Care, Community Care and Community Support Services capacity in the North West and advocate for efficiencies and system improvements.
  • Support the planning, development, and implementation of Emergency Preparedness and Management Plans within the Home Care, Community Care and Community Support Services sector.
  • Address staffing and health human resource challenges to stabilize and sustain the sector.
  • Support digital innovations that improve efficiency and patient outcomes in the sector.
  • Foster continuous quality improvement, including alignment with Ontario Health’s Quality Framework and supporting the development of Quality Improvement Plans.
  • Work with Home Care, Community Care and Community Support Services providers to align their work with Ontario Health Teams.
  • Enhance patient experience by planning and supporting the implementation of leading practices for ALC care in Home Care, Community Care and Community Support Services in the North West.
  • Promote high-quality research and health system planning, providing leadership in the development of methodologies for health planning.
  • Participate in provincial planning activities related to Home Care, Community Care and Community Support Services and conduct effective stakeholder consultations to assess the impact of planning recommendations and promote consensus among stakeholders.
  • Provide advice on health-care system funding plans from a planning perspective.
  • Manage project plans and budgets for planning and integration initiatives within the sector.
  • Ensure compliance with the Occupational Health and Safety Act, organizational health and safety policies, and contribute to a culture of patient safety, equity, and open communication.
  • Support health service providers in adhering to health service accountability agreements.
  • Coordinate and promote regional initiatives aimed at improving quality, patient safety, equity, and diversity within the sector.

Here is what you will need to be successful:

Education and Experience:

  • Post-secondary education in health care, health administration, health planning, business administration, or a related field, with a Master’s degree preferred. An equivalent combination of education and experience in a health care environment may also be considered.
  • 5+ years of experience in a health care environment, leading projects, business development, health system planning, stakeholder engagement, or client services, with experience in the acute care sector preferred. Experience in Home Care, Community Care and Community Support Services is an asset.
  • Proficiency in French is an asset.

Knowledge and Skills:

  • Proven ability to apply multiple planning and analysis methodologies and provide effective recommendations.
  • Strong understanding of the current and evolving dynamics of Ontario’s health-care system, with particular experience in seniors care, community care, and hospital care policies.
  • Experience implementing quality improvement initiatives and measuring outcomes.
  • Demonstrated financial competence, including project budget management.
  • Strong communication and interpersonal skills, with the ability to collaborate with colleagues across Ontario Health and liaise with stakeholders.
  • Highly organized and capable of thriving in a fast-paced environment with multiple priorities.
  • Excellent analytical, problem-solving, and decision-making skills, exercising independence and professional judgment.
  • Strong attention to detail, integrity, and professionalism.

Employment Type: Permanent, Full-Time

Salary Band: Band 6

Location: Ontario (currently hybrid; subject to change)
