Job Summary

Come work as a Intensive Case Management Clinician with Vancouver Coastal Health (VCH)! Vancouver Coastal Health is looking for an Intensive Case Management Clinician with a Master’s degree in an allied health discipline to join the Youth Substance Use and Prevention Services team at Robert & Lily Lee Community Health Centre in Vancouver, BC.

Youth Substance Use and Prevention Services team is designed to meet the needs of youth struggling with substance and mental health concerns. Services include prevention, counselling, and treatment services. The Youth Intensive Case Management Team (ICMT) is a multidisciplinary team that provides innovative, client-centered, and culturally sensitive services to youth with complex mental health and substance use needs.

As an Intensive Case Management Clinician, you will:

  1. Provide mental health and addiction care to clients within the designated Intensive Case Management Team.
  2. Work as part of a multi-disciplinary mental health and addictions team to provide assessments, treatment, and case management for individuals experiencing urgent and emergent mental health and substance abuse problems.
  3. Partner with clients, families, and relevant clinical care partners to ensure integrated service provision across the continuum of care.
  4. Perform key duties such as intake, mental health and addiction assessments, treatment planning, crisis intervention, consultation, case management, and education.
  5. Provide consultative and direct services to clients, families, community health professionals/agencies, law enforcement, and/or schools, physicians, and Emergency Room staff.
  6. Participate in team meetings, continuing education, conferences, and/or corporate initiatives, and engage in ongoing team planning, development, evaluation, and quality improvement activities.

Qualifications

Education & Experience

  1. Master’s degree in an allied health discipline.
  2. Three (3) years’ recent related experience providing treatment services to individuals within the designated client population, or an equivalent combination of education and training.
  3. Valid BC Driver's License; local area travel may require the use of a personal vehicle.

Knowledge & Abilities

  1. Broad knowledge of mental illness and/or addictions and treatment applicable to the designated client population.
  2. Broad knowledge of substance abuse and addictions treatment.
  3. Demonstrated counseling skills using a range of treatment modalities.
  4. Knowledge of Family Systems Theory as applicable.
  5. Demonstrated knowledge of hospital and community-based resources and the ability to act as a liaison between services.
  6. Knowledge of Early Psychosis Intervention, Attachment Theory, and Crisis Intervention Theory.
  7. Demonstrated ability to provide treatment planning, crisis intervention, and case coordination.
  8. Ability to manage complex clinical and psychosocial crises in the identified population.
  9. Basic knowledge of legislation and government policies and procedures that relate to clients.
  10. Ability to conduct mental health risk assessments, and provide treatment planning and crisis intervention for Youth/Young Adults with serious mental health and addiction issues.
  11. Able to establish rapport with multi-barrier youth and young adult populations.
  12. Ability to teach clients, families, and staff.
  13. Effective communication skills, both orally and in writing, with clients and their families, colleagues, physicians, and other health care staff.
  14. Ability to establish workload priorities and adjust schedule to deal with unexpected situations.
  15. Ability to work both independently and collaboratively as a member of a multidisciplinary team.
  16. Problem-solving skills.
  17. Ability to deal effectively with conflict situations.
  18. Ability to operate related electronic equipment.
  19. Physical ability to perform the duties of the position.
  20. Basic computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet, and e-mail software.

The hours of work, including days off and work area, may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check. As per Ministry of Health policy, all health care workers in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for the offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
