The Health Information Associate supports the Health Records processing functions and other clerical duties within the Health Records Department.
Key Responsibilities
- Answer department telephone and respond to enquiries.
- Ensure all patient charts signed out of department are returned from external locations and are stored back in the correct place.
- Coordinate filing of Inpatient, Day Surgery, Emergency, and Outpatient charts/late reports.
- Print list from Cerner to verify that all discharged charts from Clinics and Day Surgery cases have been received in the department.
- Pull charts requested by nursing units and deliver chart to unit.
- Pull lists of charts for audits or month-end processing.
- Create new patient chart if no current folder or if current chart is too large.
- Retrieve old charts stored in Health Record storage area in the basement.
- Release patient information in response to requests; circle of care, workplace safety insurance board (WSIB), and personal requests in accordance with Hospital policies. Document requests in Correspondence Log.
- Track and record release of information statistics.
- Prepare and distribute incomplete chart letters to the physicians on a monthly basis.
- Track and move death charts to the Health Records storage area in the basement.
- Coordinate and assist with the Health Records Retention and Destruction processes.
- Complete daily audit, monitoring, and providing feedback to the external transcription company.
- Subject Matter Expert for the program Deliver Health.
- Respond to inquiries from Physicians regarding Dictation procedures.
- Make corrections to transcribed documents.
- Track and record monthly transcription statistics.
- Subject Matter Expert for XR Printing.
- Attend department meetings.
- Other duties as assigned.
- Audit electronic emergency encounters to ensure proper documentation.
Qualifications Required- Medical Office Assistant Diploma or equivalent program that includes Medical Terminology and Microsoft Office education.
- Minimum of one (1) year experience.
- Able to participate and contribute as a team member.
- Strong interpersonal communication skills.
- Able to coordinate multiple priorities and adapt to changing priorities.
- Able to meet deadlines.
- Excellent computer skills, including proficiency in Microsoft Office Suite (Word, Excel, Outlook).

Salary: $26.06 - $30.44 per hour
