JOB TITLE: Housing Support Worker

REPORTS TO: Housing & Outreach Manager

DESCRIPTION:

The Housing Support Worker supervises the support services and case management of clients in the community residential support program. The position’s purpose is to coordinate, implement and supervise staff to organize workflow and caseload distribution to ensure service delivery remains effective and client-centred. The Housing Support Worker will perform duties such as ensuring adherence to tenancy and program agreements while taking a person-centred approach to support services for participating clients. The Housing Support Worker will also assist the Manager, Mental Health Programs in activity reporting and data management.

SALARY RANGE: Grid 34 – HEU – Community Subsector

QUALIFICATIONS:

  1. A diploma in Human Services or equivalent education.
  2. At least 2 years recent related work experience working with vulnerable populations.
  3. OR - An equivalent combination of education, training and experience relevant to the level of work.
  4. A valid BC driver’s license and access to personal vehicle for business related purposes.
  5. Standard First Aid & CPR.
  6. Provincial Violence Prevention certificate.
  7. Harm Reduction and Naloxone certificate.
  8. Successful clearance of a criminal record and vulnerable sector check.

SKILLS AND ABILITIES:

  1. Excellent interpersonal and communication skills in English both written and verbal.
  2. Strong organizational, advocacy and time management skills.
  3. Knowledge of theory, principles and practices of Housing First Approach, trauma-informed care and harm reduction strategies.
  4. Full Case management cycle, participant interviewing skills, assessment, care planning, goal attainment, case note documentation, and discharge planning.
  5. Demonstrated expertise in dealing successfully with persons with mental illness.
  6. Highly proficient with a variety of computer software including Microsoft Office suite and HIFIS.
  7. Strong background and working knowledge of psychosocial rehabilitation processes.
  8. Demonstrated ability to establish and maintain constructive working relationships with tenants, community groups, government agencies and the public.

KEY DUTIES AND RESPONSIBILITIES:

  1. Responsible for providing one-to-one support to a caseload of up to 82 and leading case management planning, activity and reporting for client supports with a team of three. This requires a sound understanding of case management, navigating complex systems and substance use and mental health conditions.
  2. Conducts in-home and community visits within Burnaby and surrounding areas.
  3. Consult, confer, and collaborate with society team members, Social Worker, Nurse Practitioner, Property and Tenancy Coordinator and community resources to achieve excellence in client-centred care.
  4. Performs duties such as ensuring adherence to tenancy and program agreements while taking a person-centred approach to support services for participating clients.
  5. Support clients accessing needed resources (e.g., income supports, legal guidance, tenancy) and pursuing their personal goals (e.g., employment, education and training, social and recreational activities).

Job Types: Full-time, Permanent

Pay: $31.56 per hour

Expected hours: 37.5 per week
