We are currently recruiting independent contractors offering Home Support Provider services in Wetaskiwin!
Summary:
The mandate of the service is to offer supports that match the needs of the individual in order to enhance the quality of life of the person. The three areas that are emphasized in providing service to individuals with developmental disabilities are independence, well-being, and inclusion.
As a Home Support Provider, you are a vital member of this Program; without you, we would not be able to offer this amazing residential service to individuals with developmental disabilities. You offer services in your home with a focus on independence, well-being, and inclusion.
Program Objectives:
- To give persons with developmental disabilities the opportunity to live in a private home that provides respect, dignity, comfort, and security.
- To support people with their well-being, independence, and inclusion.
- To develop annual Individualized Service Plans, taking into consideration the person’s unique strengths and needs. It is the person’s service plan and he/she will be involved in the plan and goal setting.
- To provide the person with opportunities for growth and learning.
- To ensure that Home Support Providers and Service Coordinators are appropriately skilled to provide quality care and support to the individuals.
Duties:
- Provide a stable and secure home environment where the individual is a member of the household with equal rights and responsibilities.
- Help the individual develop self-esteem, self-confidence, and independence.
- Encourage individuals to maintain good self-help skills (i.e., hygiene, nutrition, domestic tasks, etc.).
- Provide a healthy environment where there is no smoking inside the home.
- Ensure the home is safe and clean (i.e., storing hazardous materials in a safe location, safe food handling and storage, and checking water temperature as required).
- If needed, monitor and encourage that medication is taken by the individual as prescribed by their doctor. If medication administration is required, the Home Support Provider will review the Medication Administration Training Handout, complete the Medication Administration Course, complete three medication shadows, and seek clarification if required.
- Assist the individual and his/her support team in completing quarterly case reviews and monthly fire drills.
- Teach the individual what to do in case of an emergency (i.e., a quarterly fire drill is completed). The Home Support Provider will review the Fire Evacuation Plan and keep a copy on hand.
- Assist and encourage the individual to participate in social and leisure activities.
- Respect the individual’s privacy and confidentiality.
- Attend all mandatory training and keep it current.
- Sign off annually on the Agency Agreement for Provision of Contractor Services agreement.
- Home Support Providers will maintain home and vehicle insurance. Vehicle insurance requires two-million-dollar liability coverage and the home requires two-million-dollar liability coverage (see Agency Agreement for Provision of Contractor Services Agreement).
- Be aware of the Individual Rights and Responsibilities (see attachments).
- Notify the Service Coordinator of any plans around moving, having an individual move to a different bedroom or having another person move into the home.
- Operate within the parameters of Catholic Social Service’s mission, core values, guiding principles, and the home support provider Agency Agreement for Provision of Contractor Services.
Requirements:
- Complete an interview with the Program Supervisor or service designate.
- Complete a home study.
- Provide and have 3 completed references.
- Submit a Vulnerable Sector Check and Child Intervention Record Check for all adults over the age of 18 residing in the home.
- Provide proof of vehicle insurance at a minimum of $2,000,000 legal liability.
- Provide proof of home insurance at a minimum of $2,000,000 legal liability.
- Sign the Agency Agreement for Provision of Contractor Services.
