Job description

Position Summary

JFSE seeks a Home Support Link Worker position (HS Link Worker) to be responsible for connecting clients to the Home Support services, assigning shifts for home support workers with clients using a scheduling platform called AlayaCare. The HS Link Worker will work collaboratively with both the Home Support and Case Management teams to ensure the highest quality of care for our clients. The position will also include some administrative support duties and data entry.

Benefits & Compensation

This is a contract full-time position, 35 hours per week, that includes being available on call to accommodate the demands of the job. Some weekend and evening on-call time will be required to address schedule and staff changes. This position has the potential to become permanent dependent upon funding. Work from home flexibility, according to policy, is possible after successfully completing the probationary period of 90 days.

Duties and Responsibilities

Reporting to the Home Support Program Manager, the Home Support Link Worker is responsible for:

  • Linking clients to the Home Support services
  • Coordinating their care plan with the Case Management team to provide client-centered service
  • Making calls to workers and clients regarding schedules and changes in service
  • Scheduling workers/clients based on need and geographic area
  • Making changes to schedules in case of cancellations, illness, vacations, and holidays
  • Updating information in AlayaCare database and processing claims
  • Human resources support related to Home Support workers
  • Participating in program development and outreach activities
  • Taking professional development courses
  • Providing additional administrative support on occasion
  • Performing additional data entry tasks to support the Home Support Manager
  • Other tasks as required

Requirements and Qualifications

  • Health Care/Home Care/Human Services experience
  • Strong organizational and time management skills with the ability to manage deadlines
  • Excellent attention to detail
  • Excellent communicator; good listening skills and ability to provide clear explanations by phone
  • Proven effective customer service skills and ability to work well within a team
  • Ability to become an expert at using the scheduling platform/database
  • Basic proficiency with Microsoft Office including Outlook, Word, and Excel
  • Some payroll and billing knowledge or experience would be an asset but not required.
