Coordinate/assign visiting disciplines to patients including but not limited to: nursing, physical therapy, occupational therapy, speech language pathology, medical social services, dietician, home health aides. Coordinates schedules between multiple providers and disciplines to assure optimal patient care, efficient utilization of services, and compliance with physician orders.
Education: High School Diploma required or GED Equivalent.
Experience: Prior home health care, referral intake, or relative work experience in health care industry required.
Required skills: Must have proven ability to interact effectively with individuals at all levels of the organization. Have strong interpersonal skills.
