Location & Hours: Based at Reserve Health Centre, approximately 22kM away from downtown Prince George, Monday to Friday 08:30 AM to 04:30 PM. For the initial few weeks for job training requirements. However, after the training period, there might be adjustments to the times and days to accommodate service delivery needs. The standard workweek consists of 37.5 hours.
General Description:
The Lheidli T’enneh Home Care Nurse (LPN) plays a vital role in enhancing our health services and fostering positive community relations. The Home Care Nurse (LPN) provides nursing care, working within the context of the client-centered, community focused vision and goals of the Lheidli T’enneh Health care program, the FNHA Home and Community Care Program and in accordance with CLPNBC Standards of Practice. The Home Care Nurse (LPN) combines their health sciences knowledge with assessment and clinical nursing skills to effectively assess and coordinate care for Lheidli T’enneh Health Care Program. The Home Care Nurse (LPN) daily reports to the Community Nurse and is managed by the Health Department Manager.
Key Responsibilities:
- Responds to concerns identified by community care nurse and medical team regarding the assessment, formulation, implementation, and evaluation of the ongoing care of the client, and ensures care is delivered as required.
- Support the Community Care Nurse in client medical care plans.
- Provide nursing services, within defined scope of practice, to patients based on patient assessment and care planning procedures.
- Maintains a safe environment for clients, significant others, and staff by adhering to established safety policies, standards and procedures including emergency procedures and infection control guidelines.
- Performs LPN duties in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the College of Licensed Practical Nurses of British Columbia, and according to organizational policies, procedures, standards, and unit specific protocols as required.
- Meets with the client to explain home & Community care services and obtain client information, and works with the Community Nurse to implement individual care plans as determined by prior assessment.
- Contributes to the development and ongoing changes to the individualized plan of care, including organizing and facilitating interdisciplinary care planning meetings and family conferences.
- Collaborates with the Health Care Team.
- Communicates the progress of the client to other members of the Health Care Team, utilizing appropriate communication tools.
- Documents and updates all relevant information about care and process.
- Provides information on home and community care programs and policies. Follows organizational protocols while responding to client complaints and other critical incidents.
- Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings, and committees.
- Reports on unsafe conditions and malfunctioning equipment.
- Ability to be flexible and willingness to adjust to work with patients in ongoing arising circumstances.
- Administer medication and observe and document therapeutic effects.
- Monitor patients’ progress, evaluate, and document effectiveness of nursing interventions and collaborate with appropriate members of health care team.
- Provide safety and health education to individuals and families.
- Connect clients to resources in the Prince George service area.
- Train in supporting medical transportation requests and work with First Nation Health Authority.
- Physical check-ins with clients and/or elders.
- The Home Care Nurse will be required to take after hours and weekend calls from the membership for health-related services.
- Performs all other nursing-related duties as required/assigned.
Skills and Qualifications:
- Member in good standing of the College of Licensed Practical Nurses of British Columbia (CLPNBC).
- Preferred qualifications: Licensed Practical Nursing from a Canadian University or recognized equivalent.
- Experience in public health or community-based setting.
- Knowledge and understanding of First Nations communities, health, and social issues.
- Knowledge and understanding of the population health approach and the social determinants of health.
- Excellent written and interpersonal communication skills and the ability to work effectively with a variety of people and circumstances.
- Self-directed with demonstrated organizational skills.
- Tolerance and sensitivity to the needs of every individual and their right to be treated with respect and fairness.
- Ability to maintain a professional demeanor and confidentiality.
- Work with people sensitively, tactfully, diplomatically, and professionally at all times.
- Good work ethic and positive team attitude.
- Valid Class 5 Driver's License and own vehicle.
- Complete Criminal Record Check.
- Willing to undergo a criminal and vulnerable sector check.
