Job Description
In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Under the supervision of the Public Health Manager or designate, the Health Outreach Worker focuses on providing low barrier support and care to clients who may have difficulty accessing health care services through traditional modalities. The Health Outreach Worker is responsible for developing and maintaining relationships with underserved clients to ensure they receive the support and care that they need.
As a member of a multidisciplinary team, the Health Outreach Worker provides advocacy and support services for clients experiencing vulnerabilities in accordance with an established plan of care. The Health Outreach Worker reports on clients' progress and condition, including reactions to medications.
Travel is a requirement of this position. Transportation arrangements must meet the operational requirements of Island Health in accordance with the service assignment and will require the use of a personal vehicle.
Diploma in Community Social Services, e.g. Mental Health Worker, Community Health Worker, or Social Services Worker, or an equivalent combination of education and experience. Current Level C Basic Life Saving Certificate. Valid BC Driver's License. A minimum of two years recent related experience working with persons with vulnerabilities, and/or marginalized populations. Experienced working with Indigenous groups.
