Reports to Program Manager. The Network Coordinator ensures the coordination of Health for Two activities in the assigned network, through building relationships with network partners, collecting data, maintaining communication between the health for two team and agencies, promoting collaboration, and responding to needs of network partners in relation to delivery of the Health for Two Prenatal Program.

Position Responsibilities


  1. Facilitate program delivery by network partners through promoting relationships with and amongst partners, organizing regular meetings of partners (including agenda preparation and minutes), identifying successes and issues, problem-solving, and maintaining regular contact with agency partners and referring partners. This position also works to build capacity among partner agencies to support pregnant women participating in the Health for Two Program.
  2. The network coordinator is the first line of contact for partner agencies and referring agencies. It is also the networker's responsibility to return calls for potential clients who would like to join the Health for Two program.
  3. Ensure that data is collected and updated in a timely manner using the REDCap platform and AHS, such as managing duplicates and incomplete questionnaires.
  4. Ensure that network partners have resources, such as bus tickets, gift cards, educational material, and information required for program delivery (including orientation of new partners or partner staff).
  5. Promote the Health for Two program by developing relationships with related programs and services in the network area, identifying and connecting with possible partner and referral agencies, and responding to inquiries from potential partners and related agencies.
  6. Contribute to regional planning and program delivery through participation in a team of Network Nurses, Support Workers, Team Leads, Network Coordinators, the Program Coordinator, Resource Nurse, and Program Manager as well as other committees as appropriate.
  7. Assist in developing new Health for Two mom groups at different agencies.
  8. Develop and maintain excellent relationships with Alberta Health Services partners, prioritizing the partnership and maintaining a positive outlook on our partners.

Qualifications
  1. Degree in social work or Human Services field. Candidates with relevant and related experience/education will be considered.
  2. At least two years of directly related experience working collaboratively to deliver a community program.
  3. Police Check with the Vulnerable Sector and Child and Youth Intervention Check completed within the past six months.
  4. Valid First Aid certificate or enrolled in a future course.
  5. Valid Class 5 Driver’s license and reliable vehicle.
  6. Suicide Intervention training.
  7. Non-Violent Crisis Intervention training.
  8. Safe Food Handling certification (may be provided as on-the-job training).
