Position Description
POSITION: HEALTH CARE AIDE
REPORTS TO: Health & Wellness Manager
SUPERVISES: Not Applicable
Accountability & Objective
Under the direction of the Health & Wellness Manager, the Health Care Aide is responsible for performing personal assistance tasks and medication administration efficiently and cooperatively, ensuring adherence to the principles of choice, independence, individuality, respect, dignity, and a homelike environment.
Background/Qualifications
Graduate of a recognized PCA/HCA program which includes a medication administration course or have taken a separate medication administration course.
Ability to work with minimum supervision and to evaluate and meet the residents’ service needs, demonstrating initiative and problem-solving skills.
Ability to maintain effective oral and written communications (in English) and have a positive self-projection, nurturing excellent human relations with residents, families, and other team members.
Experience with seniors is preferred.
A person with a positive self-projection and ability to use initiative in problem-solving.
A people-person with a heart for seniors and a positive, outgoing disposition. Ability to work independently as well as taking direction; a team player: flexible and versatile.
Key Responsibilities
Perform duties with an acceptable standard of skill and sense of responsibility, providing personal care and service to residents.
Assist with medication delivery and ensure required documentation of all medications, including pouched medications, eye/ear/nose drops, creams, ointments, patches, inhalers, and suppositories.
Observe and report changes in residents’ physical and emotional conditions.
Participate in the overall activation process developed by the residence to promote the well-being of each resident (physical, spiritual, social, and emotional).
Perform personal assistance to promote comfort and safety for residents in accordance with the standards of the residence.
Maintain effective oral and written communication and good human relations with residents, families, and staff, using appropriate lines of communication.
Recognize safety hazards and assist in maintaining a safe, neat, clean environment for residents and personnel, reporting any unsafe work conditions immediately.
Actively monitor all residents regularly to ensure their safety and well-being, reporting any unusual occurrences, concerns, or changes in a resident’s daily life affecting their health or welfare, and documenting these incidents on a “CQI Incident Report.”
Seek out in-service education opportunities within the residence to improve personal practice and stay informed of current trends.
Maintain a good attendance record.
Participate in the Residence’s ongoing fire prevention, pest control, Continuous Quality Improvement, Occupational Health and Safety, and infection control programs as required.
Handle all matters confidentially.
Perform other duties as assigned, including assistance in other departments with an exemplary standard of skill and sense of responsibility.
