Pine River Institute (PRI) launched an innovative BRANCH program (Building Resilience and Nurturing Connections Home). This program addresses the gaps in services often experienced by youth and their families following a crisis requiring hospital contact. It will also provide caregivers with the knowledge and skills to support their youth through therapy and educational sessions. The objective of this program is to catch youth and their families immediately after a young person’s admission to a hospital or emergency room due to mental health or substance use concerns. PRI collaborates with hospitals in the GTA to establish a pathway for care beyond the hospital walls.

Reports to: Centre of Family Initiatives Director

Start date: January 6, 2025

Location: Toronto, Ontario with frequent travel throughout the city

Wage: $85,000-$90,000

The ideal candidates will possess strong leadership, communication, and positive role modeling skills. These skills will be demonstrated through a combination of educational and therapeutic sessions aimed at building resilience, where youth and their families can grow together while acquiring valuable skills for long term change.

Responsibilities (include, but are not limited to):

  1. Complete intake and assessments with youth as part of treatment team.
  2. Work with populations at risk for self-harm, suicidal ideation, and substance use.
  3. Develop, document, and deliver individual therapeutic treatment plans for a caseload of up to 15 youth and their families that are consistent with and supportive of the Institute’s program. Adjust these treatment plans over time to ensure they are focused on obtaining the treatment objectives.
  4. Work collaboratively with referring hospital.
  5. Provide in person treatment through a variety of modalities including individual therapy, consultation, case navigation/management, crisis intervention, community outreach and advocacy for youth and their families post psychiatric hospitalization for a period of 16 weeks.
  6. Support and teach relevant client and family coping skills and provide support around adaptation, treatment adherence and enhancement of quality of life.
  7. Plan and facilitate therapeutic group programming as part of treatment team.
  8. Ensure the safety, both physical and emotional, of the youth, advising and managing crisis situations as necessary, determining threshold for escalation to Senior Management and/or emergency services.
  9. Ensure the appropriate, accurate and timely flow of reports and communication with parents and students, team staff, and PRI community as necessary.
  10. Develop and sustain appropriate interpersonal relationships with youth, direct reports, peers and supervisors and community stakeholders to ensure open, constructive communication critical to the success of the program.
  11. Engage in regular supervision with designated PRI supervisor.
  12. Participate in an on-call rotation that involves providing clinical guidance to staff/youth and families.
  13. Participate in team meetings and training.
  14. Provide clinical and developmental supervision to CYW regarding youths’ needs and their therapeutic treatment plans.
  15. Work with research on developing and implementing evaluation metrics.

Qualifications, Skills, and Experience:

  1. Completed graduate degree in a relevant therapeutic field.
  2. Registration with a relevant professional college.
  3. Minimum of 5 years related experience, with experience working with crisis interventions and/or hospital setting.
  4. Demonstrated ability in providing individual and family therapy.
  5. Experience in working with adolescents, addictions, and mental health issues.
  6. Familiarity with Relational, Systemic, and Experiential approaches to working with youth and their families.
  7. Previous experience with treatment planning is essential.
  8. Demonstrated track record of working effectively with professional, collaborative groups.
  9. Proven ability to make and sustain excellent interpersonal relationships.
  10. Strong, flexible, and results-driven style.
  11. Excellent demonstrated leadership skills.
  12. Strong written and verbal communication.
  13. Clear certification of Vulnerable Sector Criminal Check is required.
  14. Ability to travel within city limits.
  15. Flexibility in scheduling that suits client needs in person, will require 2 evenings/week.
  16. Openness to engaging in regular training opportunities related to job description.
