Title: First Line Prevention Intake Coordinator

Summary of Function:
The intake coordinator will serve a pivotal role in the delivery of Listuguj Community Social Services Directorate (LCSSD) as the first point of contact for referrals from various community agencies/directorates. The Intake Coordinator will direct/triage service for families (i.e., home visitation/case management). We are seeking a motivated, dedicated individual with an interest in data entry/maintenance of both administrative and clinical responsibilities. Previous experience with intake coordination, home visits, and community-based service knowledge is strongly recommended. Knowledge in child maltreatment, family issues, and a strong sense of confidentiality is essential.
Job Duties:
- Organize, plan, analyze, and prepare for case management meetings/referrals to relevant community resources as needed.
- Provide information to families on the role of First Line Prevention Support Service (FLPSS) and the obligations of the family in working collaboratively from the FLPSS model.
- Ensure that referrals are directed to the Manager of Clinical and Family Services (Youth Protection), Manager of Prevention Services, and Family Centered Circle Manager as required.
- Maintain and update database and statistical management for reporting purposes.
- Possess strong communication and evaluation skills to interview clients and obtain case history and background information.
- Assist clients in sorting out options and developing plans of action while providing necessary support and assistance.
- Prepare intake reports monthly, ensuring follow-up is maintained, and files are opened and closed in a timely manner.
- Be knowledgeable of community agencies to liaise with partners and identify additional or alternative services, providing referrals, information, and resources to navigate social services.
- Assist with administrative tasks, address intake discrepancies within the FLPSS organization, and assist management in improving intake procedures.
- Collaborate and report to the Manager of FLPSS and/or Manager of Clinical and Family Services.
- Maintain a supportive and non-judgmental environment for clients.

Requirements:
- Minimum of two years’ experience in human services program delivery or a Youth Program Diploma or related field of studies.
- Some case management or related work experience within the social services field is appreciated.
- Strong written and communication skills as well as proficiency in word processing systems.
- Ability to pass a criminal background check.
