Job description

Responsibilities:

  1. Establish trusting and supportive relationships with assigned individuals.
  2. Maintain caseload as assigned.
  3. Provide intensive in-home services to Youth/Families identified by the Ministry of Social Services and EHC.
  4. Attend Case Management meeting(s) with Ministry of Social Services and with YM Team.
  5. Work within the Child and Family Services Act guidelines and principles.
  6. Assist in linking and supporting youth/families to access community services and resources.
  7. Provide practical support that enables youth/families to meet their goals, including transporting to appropriate appointments and recreational services.
  8. Assist with group/parenting/life skills development, planning, and facilitating.
  9. Support and encourage the development of skills such as problem solving, daily/life/coping skills, money management, education planning, and job seeking skills.
  10. Prepare daily, weekly, and monthly written reports in a timely manner.
  11. Attend all staff meetings, case management meetings, team meetings, and any other internal/external, in-service training/professional development meetings as requested.
  12. Adhere to emergency/critical incident protocols and procedures.
  13. Adhere to confidentiality and conflict of interest guidelines and duty to report policies.
  14. Provide crisis management, act as a liaison, advocate, and provide transportation when necessary.
  15. Assist in the facilitation of life skills relevant groups & sessions.
  16. Be willing to work evenings, weekends, participate in community events, fundraising activities, and other related duties as assigned.
  17. Provide Criminal Record Check.
  18. Provide a Drivers Abstract with a current Drivers License.
  19. CPR/First Aid Certificate.
