About This Role

As an Enrollment Specialist with myPlace Health, you’ll have the special opportunity to help bring our PACE (Program of All-Inclusive Care for the Elderly) services to underserved communities across Los Angeles County—ensuring that older adults can live fuller, more independent lives with dignity and support.

In this vital role, you’ll be the warm and welcoming bridge between prospective participants, their families, our Outreach Specialist, and our caring Interdisciplinary Team (IDT). You’ll guide new participants through the enrollment process with compassion, clarity, and purpose—making sure they feel supported every step of the way.

What Does Success Look Like As A Enrollment Specialist?

  • Collaboration & Teamwork: Partner with our Outreach Specialist to create a seamless and successful enrollment experience.
  • Interdisciplinary Coordination: Work closely with our Interdisciplinary Team (RN, LVN, Provider, Home Care, and Transportation Coordinator) to ensure participants have a smooth and supported transition into the program.
  • Attention to Detail: Maintain up-to-date documentation in our CRM system and ensure every record is accurate and complete.
  • Regulatory Compliance: Follow all DHCS and CMS regulations and policies to ensure ethical marketing and enrollment practices for our PACE program.
  • Goal-Oriented Mindset: Collaborate with the Outreach Specialist and Center Director to meet and exceed enrollment goals, bringing new participants into our caring community.
  • Participant Retention Support: Assist the Center Director with retention strategies or serve as an engaged member of the retention committee to help our participants continue to thrive.
  • Organizational Support: Request necessary medical records from participant providers as needed by the Enrollment Team.
  • Community Engagement: Attend community events with the Outreach Specialist to share the PACE program and connect with potential participants.
  • Process Excellence: Ensure timely processing of enrollments while meeting all documentation and procedural standards set by DHCS and CMS.
  • Audit Readiness: Participate in all PACE audits and help uphold our commitment to transparency, accountability, and excellence.
  • Relationship Building: Provide warm and informative phone screenings, conduct engaging tours of our facility, and make thoughtful home visits to help prospective participants feel welcome and informed.

What Does An Ideal Candidate Look Like?

  • Healthcare Collaboration Experience: You’ve worked alongside medical professionals and understand how to coordinate with clinical teams in a respectful and effective way.
  • Geriatric Care Knowledge: You’re familiar with intake processes and geriatric assessments, and bring sensitivity and insight when working with older adults.
  • Case Management Expertise: You’ve provided thoughtful, person-centered case management services and know how to support individuals through complex systems.
  • Clear & Compassionate Communication: You have excellent written and verbal communication skills and can connect meaningfully with both internal teams and external participants.
  • Tech-Savvy & Organized: You’re comfortable using CRM software to keep track of your work and maintain detailed, accurate records.
  • Experience with Older Adults: You’ve worked with frail elderly populations—ideally for at least one year—and understand the care, patience, and flexibility required.
  • Self-Starter with Heart: You’re highly motivated, can work independently with minimal supervision, and bring positivity and purpose to your day-to-day work.
  • Educational Foundation: A bachelor’s degree in Healthcare, Business Administration, or Marketing is preferred, but we value relevant experience and a passion for learning just as much.
  • Reliable & Ready to Drive: A valid California driver’s license, reliable transportation.
  • Language Skills are a Plus: Bilingual abilities are highly valued—especially English/Spanish, English/Cantonese, English/Mandarin, or English/Vietnamese—to better serve our diverse community.
  • Committed to Safety: Proof of current CPR/BLS certification is preferred—or you’re willing to obtain it within 60 days of being hired.

Pertinent Information For This Role

  • Your Schedule: Monday through Friday, 8:00 AM to 5:00 PM—giving you evenings and weekends to rest, recharge, and spend time with those you love.
  • Where You’ll Be: You’ll get to spend your days out in the community, serving participants in Compton and the surrounding areas. It’s a wonderful way to stay connected to the people we care for and the neighborhoods they call home.
  • Hybrid Flexibility: This role offers a nice blend of being out in the field and working from home—so you can stay engaged, energized, and balanced throughout your week.
  • We’ve Got Your Back: Mileage reimbursement is provided for all work-related travel, because we know every mile matters when you’re making a difference.

$22.12 - $31.10 a year

myPlace Health offers a robust compensation package for this role that includes cash compensation and other total rewards. Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
