About the ABI Transitional Housing Program

This is the first program of its kind in BC - one that provides a healing, client-driven transitional location for people after their acute brain injury and hospitalization. As part of an interdisciplinary team in a community program you will care for clients throughout their stay including transitions and outreach to the community. The ABI program emphasizes a supportive and innovative environment to learn and expand your knowledge and expertise. As the Licensed Practical Nurse you are passionate about helping those most impacted by the housing and toxic drug supply crises and working within a team-based care environment that emphasizes trauma informed, harm reduction and culturally safe care. As an LPN at Vancouver Coastal Health you will:

  • Provide care to clients and families based on predictable outcomes and the needs/outcomes can be managed by following an established plan of care or under the direction/supervision/guidance of a Registered Nurse (RN).
  • Work closely with the RN in direct care activities such as assessing, planning, implementing, evaluating and documenting client care.
  • Support clients and families to achieve and maintain optimal health.

Qualifications

Education & Experience

  • Graduation from a recognized program for Practical Nurses.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years recent experience working as a Licensed Practical Nurse providing care in the community or acute care setting or an equivalent combination of training, education and experience.
  • Valid B.C. Driver’s License may be required as local area travel may require the use of a vehicle.

Knowledge, Skills & Abilities

  • Comprehensive knowledge of other health care disciplines and their role in client care.
  • Demonstrated ability in dealing with a variety of situations and responsibilities requiring initiative, creativity and professional judgment.
  • Knowledge and experience in providing culturally safe and competent care, specifically pertaining to Aboriginal people.
  • Knowledge and practice within the standards dictated by the BCCNM for LPNs.
  • Broad knowledge of working with clients and families across the life span.
  • Demonstrated ability to communicate effectively both verbally and in writing.
  • Demonstrated ability to effectively deal with conflict situations.
  • Demonstrated ability to plan, organize and prioritize work.
  • Physical ability to perform the duties of the position.
  • Knowledge and experience with basic computer programs (Word, Microsoft Outlook).

The hours of work may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections.
