Job description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Manager, Pharmacy Clinical Programs, or designate, the Clinical Pharmacist, Community Health Services works collaboratively with the Seniors, End of Life, and Home & Community Care teams. The role is responsible for practicing client-centered care by identifying and resolving potential drug therapy problems in the community setting; providing medication counseling and drug information to clients and health care providers; conducting clinic sessions and/or home visits as required; and providing academic detailing to optimize prescribing practices.

QUALIFICATIONS: Education, Training And Experience

Licensed by, or eligible for Licensing by, the College of Pharmacists of British Columbia. Completion of an Accredited Pharmacy Residency and three (3) years recent pharmacy practice experience, including community-based practice or an equivalent combination of education, training and experience.

  • Ability to identify and resolve drug therapy problems for elderly and palliative clients.
  • Ability to apply clinical knowledge and evidence-based decision-making.
  • Ability to establish and maintain effective working relationships with Health Care Practitioners and care providers, as well as clients, family members, volunteers, and the public.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to be self-directed, organized, prioritize workload, and consistently complete tasks with accuracy, precision, and dependability.
  • Ability to teach.
  • Ability to be flexible and adapt pharmacy practice to a variety of community settings.
  • Ability to physically carry out the duties of the position.
  • Valid BC driver's license.
