JOB TITLE: Health Promoter
STATUS: Regular Full-time (35hr/week)
REPORTS TO: Director, Community Development Services
PAY SCALE: $35.649-41.94/hr

Job Summary
Through the lens of the Social Determinants of Health, the Health Promoter develops, implements, and evaluates health promotion initiatives aligned with the organization’s community health priorities. The Health Promoter helps to build inclusive and healthy communities by addressing barriers to health equity and fostering engagement in priority populations. This role focuses on capacity building, education, and advocacy to empower individuals and communities to take control of their health. The Health Promoter collaborates with residents, community partners, and stakeholders to support initiatives that promote well-being, prevent illness, and enhance access to resources. This position requires strong interpersonal, facilitation, and advocacy skills to drive meaningful community change.
Job-Specific Responsibilities
Community Development and Engagement:
- Develops and implements community health promotion strategies that empower individuals and communities.
- Engages priority populations to identify health and social issues, facilitating community-led solutions and promoting ownership.
- Supports the formation and sustainability of resident-led and staff-supported community groups.
- Conducts needs assessments and environmental scans to guide program planning and strategy development.
- Acts as a resource for community members, providing connections to relevant agencies, programs, and services.
- Facilitates conflict resolution strategies and coordinates crisis and post-incident responses, including responses to hate crimes and violent incidents.
Program and Project Management:
- Leads the coordination of multi-partner wraparound service models for youth and families.
- Organizes and convenes a Community of Practice among service providers, including Community Health Centres.
- Develops and updates tools and tailored supports for the early identification of at-risk individuals.
- Creates opportunities for knowledge exchange, best practices sharing, and service improvement among stakeholders, including residents.
- Facilitates city-wide and community-led discussions on safety, well-being, and local health concerns.
Service Coordination and Referral Support:
- Makes and follows up on referrals to health and social services, ensuring seamless access to appropriate supports.
- Connects individuals and families to learning opportunities, such as city-wide training sessions, community conversations, and wraparound services.
- Works closely with PQCHC staff and system navigation teams to develop integrated support strategies for community members.
Evaluation and Monitoring:
- Designs and implements evaluation frameworks to assess program effectiveness and community impact.
- Monitors project outcomes in alignment with funder requirements and organizational priorities.
- Stays informed on emerging trends in health promotion and community development to inform program improvements.
Human Resources and Volunteer Support:
- Supervises, evaluates, and supports direct reports, ensuring adherence to organizational policies.
- Provides mentorship and oversight for volunteers and placement students.
- Consults with leadership on human resource-related issues as needed.
Finance and Administration:
- Monitors program budgets and ensures compliance with financial and reporting protocols.
- Supports administrative processes, including payroll, benefits, and leave coordination, in collaboration with finance and HR teams.
Partnership Development and Collaboration:
- Strengthens partnerships with Community Health and Resource Centres and other service networks to enhance service delivery.
- Works with Community Houses, community developers, and health promoters to identify and support at-risk youth and families.
- Leads city-wide Concentric Circles of Care initiatives to improve holistic and coordinated community care.
- Participates in broad-based community planning efforts to enhance local health and social support systems.
Qualifications
- Masters or Graduate degree in health or social sciences (psychology, social work, criminology) or an equivalent combination of education and significant related experience.
- Three to five years of experience in community development/health promotion, crisis intervention, and/or social work.
- Excellent knowledge of community development values, principles, and political processes.
- Experience working with multicultural communities, low-income communities, and high-risk families.
- Demonstrated experience in group facilitation and leadership development.
- Experience working with high-risk families and knowledge of related community resources.
- Excellent organizational and communication skills.
- Fluency in English essential. Fluency in Somali, Arabic, or French a strong asset.
- Valid driver’s license and access to a vehicle is required.
- Valid First Aid and CPR (level C) required.
- Availability to work occasional evenings and weekends when required.
- Proficiency in the use of computer software applications including online communications (Telus, Zoom, Google Meets, Sharepoint, Microsoft Teams).
