Our agency provides residential support for adult males diagnosed with Autism Spectrum Disorder (ASD). We are currently looking to recruit for a full-time Case Manager.
Job Description
CASE MANAGER
Oversee the individualized support of your assigned resident(s).
Coordinate visits with parents and appointments on behalf of resident.
Direct support workers on how to best provide care to assigned resident(s).
Assist in the development of assigned resident(s) Individual Support Plans.
Complete all duties associated with the Support Worker position outside of Case Manager duties.
Required Experience
Child and Youth Worker, Developmental Service Worker or Social Service Worker diploma or related field with 12 months of working experience in the field considered an asset.
Experience in Applied Behavioural Analysis (ABA) is an asset.
Experience with behavioral management systems (i.e., UMAB) is an asset.

Required Documentation
Upon hire, you will need to provide a valid Criminal Reference Check for Vulnerable Sector, a completed medical form with updated immunizations, current First Aid and CPR certifications, and a valid Ontario G class driver’s license in good standing.
