Position Summary and Scope of Service:

Reporting to the Senior Manager, Programs and Services and consistent with the Mission, Values, and policies of St.Amant, the Manager, Adult Health Services (AHS) is a critical role in ensuring the success of St.Amant business and operations.

Exemplifying the mission, vision, and values of St.Amant, the Manager, AHS works collaboratively with partnered programs while offering employees respectful customer service.

The Manager is responsible for the operations of AHS with a focus on:

  • An interdisciplinary model of care
  • Staff development including mentorship, coaching, and performance management
  • Supporting evidence-based practice that meets current Quality and Accreditation standards
  • Coordinated flow of people supported including timely admissions, discharge planning, and escalation of barriers to discharge

The Manager participates in activities of importance or relevance to Health and Transition Services and the organization as a whole, including (but not limited to): strategic planning and initiatives, developing community partnerships, quality evaluation & improvement, risk management, and resource acquisition (human, financial, physical).

The Manager participates in the rotating on-call process for the Health and Transition Services program.

Qualifications

Educational:

  • Undergraduate degree in a relevant healthcare discipline required, nursing preferred. A combination of education and experience may be considered.
  • Certificate in Health Care Management (completed or in process) is an asset.

Professional:

  • Minimum of five (5) years’ experience in a clinical environment requirement.
  • Minimum of three (3) years’ experience in a clinical management or leadership role required.
  • Experience supporting patient/resident/client flow.
  • Experience managing a budget.
  • Experience supervising in a unionized environment.
  • Demonstrated history of effective working relationships within a collaborative, interdisciplinary team environment.
  • Valid registration with CRNM or other relevant/applicable licensing body.
  • Minimum of five (5) years’ experience supporting individuals with neurodevelopmental disabilities and complex health, behavioural and social needs, including mechanical ventilation.
  • Current awareness of trends and best-practice approaches to supporting individuals with Intellectual and Developmental Disabilities (IDD).
  • Familiarity with relevant legislation and regulations (e.g., Personal Health Information Act (PHIA), Vulnerable Persons Act (VPA), Workplace Hazardous Materials Information Systems (WHMIS), Universal Precautions, etc.).
  • Proficiency in a Microsoft Office environment.

  • Ability to work flexible and extended hours when necessary.

Duties and Responsibilities

Program/Service Support:

Consistent with the Mission, Philosophy, and Policies of St.Amant, the Manager, AHS oversees the care services, and quality of life experienced by individuals within the assigned service area. Priority is given to ensuring a person-centred and family-centred approach in care and service planning, ensuring high-quality care and quality of life in the least restrictive environment possible. Quality services are evident when professional communication, timeliness, accuracy, ethical awareness/sensitivity, and confidentiality are consistently demonstrated as required by legislation and program/service standards.

Duties include:

  • Oversees and participates in all elements of the flow process including: admission, care planning, and discharge.
  • Regularly evaluates services provided by the interdisciplinary team including, but not limited to: care planning, delivery, outcomes, and documentation.
  • Ensures medical, personal, and quality of life care meets identified standards.
  • Promotes and maintains a safe and respectful environment.
  • Communicates effectively and works collaboratively with the interdisciplinary team inclusive of the individual and family to address concerns regarding care provision.
  • Ensures the development of care team assignments and work schedules achieve individual, service area, and program objectives.
  • Ensures that all required documentation is completed accurately and within expected time frames.
  • Escalates clinical care concerns to the assigned Senior Manager as required.
  • Participates directly in care and supervision where required.
