JORDAN’S PRINCIPLE SERVICES COORDINATOR

Full-Time Permanent

Location – Kenora

POSITION SUMMARY:

The Jordan’s Principle Services Coordinator is a full-time permanent position with primary responsibility to act as focal point of contact for children and families accessing services from the Jordan’s Principle Team. The Jordan’s Principle Services Coordinator provides service coordination and planning in cooperation with other agency personnel related to children in care and children not in care of the agency and provides supervision to the Jordan’s Principle’s Administration Support clerk.

QUALIFICATIONS

  1. Bachelor of Social Work Degree is preferred, however, a combination of skills, education and experience may be considered if incumbent possesses a college diploma in Human Services and two years direct work in coordinating social programs and services for First Nation people;
  2. Thorough knowledge to plan and coordinate services for children who require AAFS Services from the Jordan’s Principle Team relating to intake, ongoing file management and case continuation or discharge in conjunction with agency programs and reporting;
  3. Knowledge to assist families in completing applications for funding;
  4. Knowledge to develop and submit any related proposals in a timely manner;
  5. Work with AAFS Finance Department for processing of Jordan’s Principle funding claims and any other secured funding;
  6. Proactively identify children with unmet needs to facilitate early intervention and timely access to services and resources;
  7. Help families navigate the health and social systems in a timely manner;
  8. Provides finance support services to all program areas of the finance and administration team;
  9. Ability to provide monthly report writing, submissions status and documentation and other as deemed necessary by AAFS and by Ontario/Federal legislation;
  10. Ability to analyze and identify gaps related to children’s needs and services for each case managed;
  11. Ability to take direction and work within the agency philosophy, policies, procedures, and guidelines;
  12. Ability to establish and maintain purposeful relationships with staff, management, and other community agencies and service providers;
  13. Strong commitment to help Anishinaabe children and their families by providing culturally competent services;
  14. Always committed to ensure highest level of confidentiality with a thorough understanding that all agency matters related to services, clients, staff, and all other agency business must be kept confidential;
  15. Must be aware of the cultural environment in which AAFS Family Services operates;
  16. Ability to flex daily work hours;
  17. Must possess a valid Ontario G Driver’s License, be willing to travel and provide a Driver Abstract;
  18. Ability to speak Anishinaabemowin is preferred and a definite asset.

Working Conditions:

The work will be normally performed in a typical interior and office environment located in the offices of AAFS. The position will require the incumbent to sit for long periods of time at a desk, walk, stand, bend, stoop, crouch and stretch. Use of arms, hands or fingers in typing, handing, carrying objects and repetitive movements. The noise level environment is usually quiet to moderate. Travel to semi-remote Anishinaabe communities on rough and winding class “C” roadways and in various weather conditions.

Salary Range: $58,476- $71,716 per annum
