We are seeking a Social Worker who is enthusiastic and passionate about opportunities to work with individuals in the
community. Our Social Workers are key members of several frontline teams, including the Transitional Rehabilitation
Housing Program (TRHP), and Assertive Community Treatment (ACT). These roles contribute to the support of our clients
in providing invaluable services to individuals with addictions and/or mental health concerns within the Kingston,
Frontenac, Lennox and Addington (KFLA) communities.

Job Summary:
The Transitional Rehabilitation Housing Program Social Worker, reporting to the Program Manager, works as a part of an
interdisciplinary team providing psychosocial rehabilitation and person-centered support to individuals within the
Transitional Rehabilitation Housing Program. The Transitional Rehabilitation Housing Program Case Manager works
independently and in conjunction with partners, carrying out treatment rehabilitation and support functions to facilitate
successful community living for Providence Care Forensic Mental Health clients under the jurisdiction of the Ontario
Review Board (ORB), using a recovery-oriented model of care for clients within the Transitional Rehabilitation Housing
Program. The Transitional Rehabilitation Housing team member aims to build the capacity and strengths of individuals
and works as an integral member in building a sense of community within the residential setting. This incumbent is
responsible for participating in the comprehensive assessment of clients, coordinating the individualized treatment plan
and required services and needs of assigned clients and their informal supports. Additionally, the incumbent is responsible
for providing rehabilitation support to these individuals, including but not limited to assistance with instrumental activities
of daily living (IADL), daytime activities, and sharing in the responsibility for addressing the needs of all clients providing in
home support and supervision. Social Work expertise is shared with other team members to assist in addressing the health
and well-being of clients and to assist in each client’s engagement and partnership with family members, accessing
entitlements, and individual advocacy to ultimately support recovery. All services are provided in accordance with the
mission and established protocols of the Agency and the principles of psychosocial rehabilitation.
This position is FLS Designated
As such, the Case Manager for French Language Services is responsible for providing services to clients in English and
French and works collaboratively within the team on supporting French-speaking clients outside of their assigned case
load. The incumbent may also be asked to support French-speaking clients of other programs of the agency as required.
Travel may be required to all KFL&A sites.
Note: If the competition is unsuccessful for a French Language Services individual, applicants without proficiency in French

Minimum Qualifications:
Oral French Language proficiency at the Advanced level, in accordance with the provincial standard. Documentation of proficiency and/or additional third-party testing/assessment is required.
Bachelor of Social Work and current unrestricted registration with the Ontario College of Social Workers and Social
Service Workers.
Three years relevant experience providing services to individuals who live with a serious mental illness, preferably in
a community setting.
Certification in First Aid and CPR.
Applied Suicide Intervention Skills Training an asset.
Crisis Prevention & Intervention training an asset.
Knowledge, Skills and Abilities:
Knowledge of and ability to apply social work principles and practices as they relate to individuals who live with a serious mental illness, within a client-centred approach.
Knowledge of the Forensic Mental Health System and the Ontario Review Board.
Demonstrated knowledge of mental health symptoms, issues, psychotropic medications and associated side effects.
Excellent bio-psycho-social and health assessment skills.
Demonstrated in-depth knowledge of mental health symptoms, issues, psychotropic medications and associated side effects.
Knowledge of issues related to addictions an asset.
Knowledge of and ability to apply principles of Psychosocial Rehabilitation and the Recovery model.
Knowledge of and the ability to incorporate best practices in community support for individuals living with a serious mental illness.
Demonstrated assessment and crisis intervention skills with broad knowledge of emergency mental health and crisis stabilization.
Demonstrated advanced clinical reasoning and decision-making skills.
Ability to effectively utilize community resources and supports to meet the needs of individuals.
Ability to work under pressure within a changing environment.
Working knowledge of the Health Care Consent Act, Mental Health Act, Personal Health Information Protection Act, and other relevant legislation.
Excellent communication and interpersonal skills in order to effectively engage community services and individuals, increasing awareness of services.
Well-developed problem-solving, prioritization and conflict resolution skills.
Ability to work autonomously as well as collaboratively in a multidisciplinary team environment.
Basic proficiency in computer skills; MS Office preferred.
Ability to work rotating days, evenings and weekends.
Must possess a valid Ontario Driver’s License and have regular access to a reliable vehicle and provide proof of $2,000,000 vehicle insurance. Incumbent may be required to transport clients.
Required to provide a satisfactory criminal reference check (CRC) with Vulnerable Sector Screening prior to hire.
