Social Worker [SLDP 01-25]

Job Summary:
Reporting to the Manager of Mental Health and Addictions Program (MHAP), Assault Care and Treatment Program (ACT), Sioux Lookout Diabetes Programs (SLDP/CCDC), the Social Worker will work closely with the interdisciplinary team within the Sioux Lookout Diabetes Program (SLDP). The Social Worker will initiate referral to appropriate personnel based on knowledge of the scopes of practice of other members of the health care team/advocates on behalf of patients and family/significant others. In consultation with the patient, family/significant others and the health care team, individualizes the multidisciplinary patient care path to reflect interventions to meet identified needs while promoting cultural appropriateness, safety, privacy and confidentiality for patient/family/significant others.

Qualifications:

1. Post-secondary education in Social Service Work, Social Work, or equivalent.
2. Registration with Ontario College of Social Workers and Social Service Workers, or eligible to obtain.
3. Innovative thinker, able to work in a result focused environment.
4. Ability to analyze and interpret the needs of clients and offer the appropriate options, solutions, and resolutions.
5. Excellent skills in all MS Office.
6. Experience in healthcare organization is an asset.
7. Exceptional conflict resolution and negotiation skills.
8. Able to respond quickly in a dynamic and changing environment.
9. Highly flexible, with solid interpersonal skills that allow one to work effectively within an interdisciplinary team, and with patients, families and communities.
10. Training or experience in cognitive Behavioural therapy, crisis or suicide intervention, and/or motivational interviewing is an asset.
11. Able to build and maintain lasting relationships with other departments, communities, fellow employees and patients.

Job Duties:

1. Case management services to adults and children living with diabetes, with an emphasis on addressing socioeconomic determinants of health.
2. Assess and prioritize patient needs, establishing individualized plan in collaboration with patients and families.
3. Works cooperatively and collaboratively with other members of the diabetes team and circle of care.
4. Provide services that are individualized and tailored to address patient’s current needs and preferences.
5. Maintains an updated knowledge of services provided by community resource agencies.
6. Maintains accurate patient care documentation, case management and statistics in accordance with hospital standards.
7. Advocate on behalf of patients.
8. Acts as a navigator to direct patients to sources of support by assisting with completion of social assistance applications and support letters.
9. Assist children and families to develop healthy coping skills.
10. Provide crisis intervention and referral to appropriate counselling or therapy services.
11. Maintain current diabetes information for patients.
12. Maintain communication with patients and the interdisciplinary healthcare team.
13. Travel to remote and isolated communities by car and plane is required, including overnight travel.
