JOB SUMMARY:

Carpenter Hospice is an 11-bed home in the heart of Burlington that offers resident-based care to individuals in the last stages of their lives and community-based programs for individuals dealing with grief or a life-limiting illness. It is an exciting time to be joining Carpenter Hospice as we expand our resident and community services in Burlington and surrounding areas.

Carpenter Hospice is excited to introduce a full-time opportunity for a Director of Care, who is a Nurse Practitioner (Registered Nurse in the extended class), working within their legislative scope of practice as described and outlined by the College of Nurses of Ontario. The Director of Care ensures all clinical services are delivered in accordance with standards of practice, accreditation standards for a hospice, and the appropriate regulatory and legislative requirements for a hospice. The Director of Care collaborates with the resident and family/caregiver, the hospice’s physician medical consultant, and the health care team in the development, implementation, and evaluation of the resident’s plan of care; provides leadership and mentorship to staff to enhance their knowledge, assessment skills, and ability to care for residents at end-of-life; and leads and collaborates in research, education, and evidence-based practice initiatives to optimize the resident’s experience.

KEY RESPONSIBILITIES:

  1. Participates in continuing education and maintains current knowledge and practice in the delivery of palliative care.
  2. Directs and organizes Hospice’s health care services as required by applicable legislation and policies.
  3. Provides advice and is accountable for areas of health-care services and policies in Hospice, including:
    1. Development, implementation, and evaluation of health care services.
    2. Preparation and ongoing review of health care policies and procedures.
    3. Involvement in Hospice-wide issues, such as infectious disease outbreak management.
    4. Provides other services in relation to the provision of medical services in Hospice, as jointly agreed upon with the Executive Director (or designate).
  4. Available (or designate) in case of an emergency when a resident’s attending physician/nurse practitioner or their delegate cannot be reached.
  5. Develops and maintains a policy for the provision of health care services for all residents who do not have an attending physician/nurse practitioner identified.
  6. Provides input into policies and criteria for admission to Hospice and consultation for review of applications for admission.
  7. Acts as a medical liaison with local hospitals and other appropriate health care organizations.
  8. Acts as a change agent by advocating for and implementing new initiatives.

DIRECT CARE TO RESIDENTS:

  1. Provides comprehensive palliative care to residents as part of the interdisciplinary care team.
  2. Increases continuity of care through collaboration, consultation, and referral as appropriate.
  3. Assists, supports, guides, and provides consultation to nursing staff and interdisciplinary team members regarding challenging clinical situations.
  4. Engages with residents/substitute decision makers, care team in regular dialogue about their goals of care.
  5. Orders and/or performs appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up.
  6. Participates in providing after-hours and on-call coverage in accordance with the on-call policy of the Hospice.

TEACHING AND COACHING:

  1. Provides formal and informal teaching and coaching to interdisciplinary team members.
  2. Provides clinical placements and precepts students interested in specializing in palliative care.
  3. Participates in identifying, analyzing, and interpreting trends in resident care outcomes.
  4. Contributes to planning, implementing, and evaluating learning resources and health education programs.

RESEARCH:

  1. With the approval of the Executive Director, undertakes research projects involving residents of Hospice.
  2. Engages in evidence-informed practice by critically appraising and applying relevant research.
  3. Collaborates with members of the interdisciplinary team to identify research opportunities.
  4. Acts as a change agent through knowledge translation and dissemination of new knowledge.

QUALIFICATIONS:

  1. Master of Nursing Degree – Primary Health Care Nurse Practitioner (PHCNP) or Adult Nurse Practitioner, required.
  2. Palliative Care training or certificate, required.
  3. Current certificate of registration in good standing with the College of Nurses of Ontario as a RN EC (Extended Class), required.
  4. Licensed to prescribe Controlled Substances and Narcotics, required.
  5. At least three years of progressively responsible experience in a clinical setting, required.
  6. Experience with community health in primary care considered an asset.
  7. Proficient leadership, communication, and clinical documentation skills.
  8. Competent negotiation and conflict resolution skills.
  9. Reliable transportation and willingness to travel.
  10. Provide current Vulnerable Sector Police Record Check.

Note: Carpenter Hospice does not require mandatory inoculation against influenza, Hep A + B, however we recommend these inoculations.

Pay: $115,000.00-$135,000.00 per year
