Reporting to the Health Services Manager, Primary Health Care, Chronic Disease Management and Wellness, the incumbent will provide expert knowledge and skills on a wide range of issues from advocacy to assessment to case management. The Dietitian will use critical thinking and clinical judgment in coordinating, facilitating, and improving the management of patients with Diabetes and co-occurring chronic conditions within an integrated primary health care model.

The PHC Dietitian, as a member of an interdisciplinary collaborative team, provides a range of PHC services including health promotion, illness prevention, screening, treatment, and monitoring. The incumbent will collaborate with others to increase community capacity as an educator, researcher, and clinical practitioner.

The incumbent will provide health assessment, interventions, and appropriate follow-up in accordance with acts, regulations, policies, standards, and guidelines of the Registered Dietitians of Canada, the Nova Scotia Dietetic Association, the Department of Health and Wellness - Province of Nova Scotia, and the Nova Scotia Health Authority.

About You

We would love to hear from you if you have the following:

  1. Bachelor of Science in Human Nutrition at a recognized university
  2. Dietetic internship (a 1-year integrated or post-graduate program)
  3. Annual continuing education to maintain Nova Scotia license
  4. Two years clinical experience preferred
  5. 1 year of diabetes chronic disease management experience required. If no applicants meet the one-year experience required, applicants with less than one year’s experience may be considered.
  6. Membership in Dietitians of Canada preferred
  7. Current CPR/BLS certification is required
  8. Designation as a Certified Diabetes Educator preferred, or required to complete within 2 years.
  9. Willingness to obtain adult insulin dose adjustment within 6-12 months and additional insulin dose adjustment certifications within 2 years.
  10. Experience with group education
  11. Experience providing self-management support
  12. Exhibits creative problem-solving ability; Ability to work collaboratively with other disciplines and committees to achieve excellence in patient care
  13. Computer proficiency in MS Office (Word, PowerPoint), email, and internet
  14. Ability to work well independently as well as within a multidisciplinary team environment
  15. Demonstrated commitment to diversity, equity, and inclusion
  16. Excellent verbal and written communication skills
  17. Effective research and analytical thinking skills
  18. Competencies in other languages an asset; French preferred

Hours

  • Permanent full-time position; 75 hours bi-weekly.
  • Shifts include days and may include evenings and weekends

Compensation and Benefits

$34.78 - $45.85 Hourly
