Coordinator of First Five Program
1 FTE
Thunder Bay Site

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking a 1 FTE Coordinator to join our community health centre team. In this newly funded role, the Coordinator will oversee the Preventative and Health Promotion Primary Care Program which encompasses midwifery care, lactation services, and primary health care. These programs are designed to support unattached children and their families- defined as those who are prenatal to 5 years of age and without access to a primary care provider. Based at the Thunder Bay site, this position will support ongoing prenatal and post natal care initiatives and assist with exploring options for expanding the First Five programs within the community, with an emphasis on newcomers and underserved populations.

As the leader of an inter-professional care team, the Coordinator will focus on health prevention, health promotion, primary health care delivery, health education, and building strong community partnerships. This role is a critical part in improving health equity for children and families by ensuring access to comprehensive, culturally safe and community driven care.

QUALIFICATIONS:

  1. Post-secondary education and experience in health, public health, or social service related fields.
  2. Current registration with the College of Nurses of Ontario, College of Dietitians of Ontario or the College of Social Workers and Social Services Workers is required.
  3. Experience with developing a new program, implementation of programs, evaluation ongoing and working collaboratively and effectively with interprofessional team(s).
  4. Experience in leading interprofessional healthcare teams is an asset.
  5. Knowledge and experience develop strong community and social service partnerships to promote health promotion and health prevention and health promotion primary care services to children and families with health equity discrepancies.
  6. Demonstrated values alignment with organizational vision, mission and values.
  7. Awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQI+, newcomers, and those non-insured).
  8. Exceptional written, oral and presentation skills.
  9. Exceptional leadership, organizational and time-management skills.
  10. Able to work independently and in collaboration with others.
  11. Excellent problem solving and critical thinking skills.
  12. Demonstrated commitment to the principles of equity, inclusivity, respect and cultural competence is required.
  13. A current driver’s license and access to a vehicle.
