Join Our Heart at Home Team as a Licensed Practical Nurse (LPN)!
Are you passionate about helping clients journey to better health and recovery?
Heart Pharmacy's Heart at Home (H@H) program is looking for a Full-TimeLicensed Practical Nurse (LPN) to provide compassionate care to individuals undergoing treatment for substance use through Opioid Agonist Therapy (OAT).
Your work will have a meaningful impact on patients' lives across Greater Victoria.

Schedule:
  • Scheduled available: Sunday to Thursday or Tuesday to Saturday.
  • Hours: 7:00 AM to 3:00 PM.
Your Role:
As a member of our Heart at Home team, you will:
  • Substance Use Treatment: Provide in-home and on-site care to patients undergoing OAT, ensuring adherence to treatment plans and promoting recovery.
  • Medication Management: Dispense medications, including insulin, and manage clients’ medication routines.
  • Health Monitoring: Check blood sugar levels, administer injections, and assess patient well-being.
  • Collaboration: Work closely with the Vancouver Island Health Authority and local healthcare providers to deliver seamless care.
  • Patient Engagement: Build meaningful connections with clients, becoming a familiar and trusted presence in their care journey.
What We’re Looking For:
  • Qualifications:
  • Completion of an accredited LPN program and current registration with the BCCNM.
  • Previous experience or strong interest in substance use treatment and OAT protocols.
  • CPR and First Aid certification (current and valid).
  • Skills:
  • Exceptional communication and interpersonal skills.
  • Basic computer proficiency for documentation and reporting.
  • A deep commitment to compassionate and patient-centered care.
  • Requirements:
  • A valid BC driver’s license, a reliable vehicle, and a clean criminal record check.
  • Ability to navigate stairs and perform light physical tasks as required by in-home visits.
