LAMP Community Health Centre (LAMP) is partnering with Roots Community Services Inc (Roots CS) and the Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) to design, develop and implement an integrated health and social services hub with and for the Black, African and Caribbean (BAC) communities in Peel Region.

The Hub will provide primary health, mental health, addictions and social services programs that will holistically address the physical, mental, social and cultural needs of BAC communities, and seek to eliminate disparities and inequities.

To achieve our goals, we are recruiting people who are passionate about delivering culturally relevant, responsive and affirming care that will inspire individuals to improve their wellbeing.

The Black Health and Social Services Hub is seeking a Community Health Worker focused on Preventative Care for BAC Communities living in Peel Region. This position is focused on providing upstream and proactive health care for individuals and communities through locally informed, culturally responsive interventions to improve access to health and wraparound, health and social service supports. This position is reporting to the Clinical Director, BHSS Hub.

Primary Responsibilities

  • Plan and deliver activities for BAC communities, including community group programming, to support the development, implementation, and evaluation of health prevention initiatives.
  • Maintain an understanding of the health and social service needs of BAC community members through data and research, and utilize this understanding to inform the planning and delivery of group programs for BAC communities.
  • Utilize a culturally safe approach to provide client-centered screening, consultation and assessment to identify opportunities for referral to primary care, mental health, addictions, and social services.
  • Conduct outreach across the Region of Peel, which includes attending community events, and forming relationships with organizations that provide care to BAC community members.
  • Work in collaboration with the BHSS Hub team to identify resources (both internal to the Hub, in Peel Region, as well as within the Greater Toronto Area) that will enhance BAC community member capacity to meet their goals and self-manage health needs.
  • Attend stakeholder meetings where intake and service navigation matters are addressed, providing solutions and following up as appropriate in a timely manner with all concerned parties and nurture those relationships as a trusted representative of LAMP CHC and the BHSS Hub.
  • Contribute to overall team functioning by assisting where needed and with the development of quality and process improvement initiatives.
  • Advance the Hub’s work by participation in activities that seek to strengthen collaborative and interdisciplinary teamwork, such as interdisciplinary committees.
  • Maintain and develop new professional competencies as directed by their supervisor.
  • All other tasks and duties as assigned by the Supervisor/ Manager.

Required Qualifications

  • 3+ years of relevant experience in community healthcare, health education and program development.
  • Educational background in health studies, such as social sciences, health science, or health administration.
  • Experience working with high priority and at-risk communities, with knowledge of relevant services.
  • Demonstrated problem-solving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, inter-professional team environment.
  • Ability to work well under pressure in a fast-paced environment and be adaptable to the changing needs of the organization.
  • Detail-oriented with ability to multi-task, organize, prioritize, and meet deadlines.
  • Strong problem-solving skills and attention to detail and accuracy.
  • Solid working knowledge and ability to navigate a PC windows environment, including shared drives.
  • Strong skills and experience using Microsoft applications (Word, Outlook, Excel, PowerPoint & Publisher) and navigate the internet/intranet environment.
  • Experience and accuracy using database applications (i.e. Electronic Health Records –Nightingale, funder databases, etc.)
  • Ability to learn new software quickly and willingness to continuously develop technology skills.
  • Knowledge of community resources in the Region of Peel is an asset.
  • A second language that is reflective of the Black, African, Caribbean communities being served is also a preferable asset.

Working Conditions

  • The hours may vary (day and evening), with shifts predominantly in the afternoon and evening based on community need.
  • Manage clients with complex social and medical needs.
  • May require independent work with variability in interdisciplinary supports.
  • High caseloads - must be able to multitask, work in a fast paced, multi-faceted environment and make efficient decisions.
  • Travel to multiple locations for clinics, mobile clinics, and outreach. Driver’s license (G or G2) and vehicle is an asset.

Physical Requirements

  • Data entry, with long periods of time sitting stationary in front of a computer monitor while keyboarding.
  • Ability to balance all job requirements in a busy atmosphere, including responding promptly to all communication.
  • Frequent standing and sitting for extended periods of time and some occasional lifting may be required.
  • Demonstrate good and safe work habits, and maintain a clean working environment.
  • Ensure full compliance with LAMP’s Health and Safety regulations and LAMP’s policies and procedures.
