The Community Health Navigator is responsible for participating in the delivery of health programs to TH citizens that promote health education, awareness, prevention, and healthy lifestyles. This role aims to protect, prevent, and reduce the incidence of cancer and chronic conditions, communicable diseases, injuries, disabilities, and premature passings. As a member of the Wellness team, the Community Health Navigator serves as an advocate for citizens and liaises between patients, families, Yukon Government health professionals, and additional partnerships when required. This is achieved through community health surveillance, effective health programs, and initiatives, using a collaborative case management approach that includes TH culture and traditions in health service delivery.
An Eligibility List may be established from this competition – the duration of the eligibility list may be up to 12 months. The eligibility list may be used to fill future vacancies within the same department and classification based on the organizational needs.
