Salary: Level 7 Step 1 ($3,316.50 biweekly)

The Community Health Navigator is responsible for participating in the delivery of health programs to TH citizens that promote health education, awareness, prevention, and healthy lifestyles. This role aims to protect, prevent, and reduce the incidence of cancer and chronic conditions, communicable diseases, injuries, disabilities, and premature passings. As a member of the Wellness team, the Community Health Navigator serves as an advocate for citizens and liaises between patients, families, Yukon Government health professionals, and additional partnerships when required. This is achieved through community health surveillance, effective health programs, and initiatives, using a collaborative case management approach that includes TH culture and traditions in health service delivery.

An Eligibility List may be established from this competition – the duration of the eligibility list may be up to 12 months. The eligibility list may be used to fill future vacancies within the same department and classification based on the organizational needs.

Essential Qualifications:

  1. Post-Secondary Certificate / Diploma in Public Health, Nursing, Nutrition, Social Work, or a related field. An equivalent combination of education, training, skills, and experience may be considered.
  2. Experience in community-based health, social, or wellness programs.
  3. Knowledge of, or willingness to learn, Tr’ondëk Hwëch’in government structure, culture, traditions, history, demographics, family structures, and socio-economic dynamics.
  4. Ability to plan, organize, implement, deliver, and evaluate health-related programs.
  5. Coordinate programs and obtain participation from citizens and other agencies.
  6. Computer proficiency.
  7. Provide advice and guidance on health care services to the community and staff.
  8. Ability to develop and maintain positive relationships with the community, citizens, families, co-workers, and other departments.

Conditions of Employment:

  1. Criminal Record Check with Vulnerable Sector Screening.
  2. Class 5 Driver’s License with Clear Driver’s Abstract.
  3. Standard First Aid certification or willingness to attain.
  4. Foodsafe Level 1 certification or willingness to attain.
  5. TH101 cultural awareness.
