CMHA Kelowna is hiring a full time Case Manager in the Housing First for Youth Program. This position is 37.5 hours a week with a typical work week of Monday – Friday 8:00am – 4:00pm.
WHO IS A CASE MANAGER?
The Housing First for Youth (HF4Y) program is designed to address the needs of developing adolescents and young adults by providing them with immediate access to housing that is safe, affordable and appropriate, and the necessary and age-appropriate supports that focus on health, well-being, life skills, engagement in education and employment, and social inclusion. The goal of HF4Y is not simply to provide housing stability, but to support and facilitate a healthy transition to adulthood.
As the successful candidate, you will be responsible for providing intensive Case Management services and support to a small caseload of youth (7-10 maximum) that are experiencing, or at significant risk of, being unhoused. Following a fidelity based HF4Y program model, you will support young people in finding and maintaining housing, connecting them with necessary and complimentary professional and natural supports, with the goal of ending their experience of homelessness, preventing future episodes of homelessness, supporting recovery and connection to community. Services will take place in the community and in the young person’s home.
