Case Manager – Bridgetown-Greenbushes and Manjimup Locality

Blackwood Youth Action (BYA) is the Warren Blackwood region’s leading provider of youth mental health services. We have an impressive record of responsive, innovative and effective service delivery and of achieving noteworthy outcomes for vulnerable youth in our region.

If so, we are looking for someone:

  • Who has, or is studying towards, a tertiary qualification in Social Work, Psychology or a related field, or who has relevant specialised skills and experience sufficient to perform at the required level.
  • Who has proven their ability to be versatile, meeting the complex demands of developing, implementing and delivering successful case management.
  • Who wants to work in the town of Bridgetown-Greenbushes and the Manjimup locality in WA’s southwest, with travel within the Blackwood River region.
  • Who is looking for a full-time contract position of 76 hours per fortnight (1.0 full time equivalent) under the Social, Community, Home Care and Disability Services Industry (SCHADS) Award.

About the Role

The Case Manager provides specialist, comprehensive, diverse case management services to young people (12 to 25 years of age) who are resident in the Bridgetown-Greenbushes and Manjimup locality of Western Australia. The Case Manager works collaboratively with the community, creating linkages, networks and appropriate referrals to deliver and achieve client-centred outcomes.

Key Responsibilities and Duties

  • Provide one-to-one high-quality case management, informal counselling and advocacy for young people who have been identified as ‘at risk’, and/or requiring additional support.
  • Establish and maintain a therapeutic alliance which focuses on increasing resilience, knowledge of coping strategies and readiness for therapy.
  • Undertake intake process by reviewing referrals, completing intake interviews and assessing suitability to BYA case management and programs.
  • Develop, implement, deliver and evaluate individualised case plans with young people in a strengths-based and youth-centred manner that promotes opportunities for recovery, and adoption of mentally healthy behaviours.
  • Liaison, advocacy and relationship building with support agencies, schools, local hospitals, police and youth related community groups.
  • Provide in-reach support to young people within educational settings.
  • Maintain client records and case management databases, by completing and updating case notes and logging events and progress in an accurate, timely and confidential manner.
  • Review and consider client progress, obstacles and opportunities for case management improvements in team interdisciplinary meetings.
  • Prepare reports and statistics as required by analysing, and summarising outcomes, data and trends.

Essential Qualifications, Skills and Attributes

  • Relevant tertiary qualification in Social Work, Psychology or related degree AND/OR substantial experience in relevant service stream with specialised skills sufficient to perform at the required level.
  • Experience in the design, implementation, delivery and evaluation of case management action plans to achieve outcomes.
  • Able to work both independently and as a member of a team.
  • Excellent written and oral communication and advocacy skills.
  • Current C-class driver’s license and roadworthy vehicle.
