Nodin Mental Health Services is Sioux Lookout First Nations Health Authority’s mental health department. It is a resource available to First Nation children, youth, adults and families from 33 remote northern communities served by SLFNHA. Nodin MHS has several services: intake, counsellors travelling to communities, a crisis response program, community-based children’s mental health and addiction workers, traditional healing, on-call, youth school counsellors, and an outpatient mental health service (OMHS).
The Case Manager is a member of the OMHS multi-disciplinary team. This team provides intensive brief counselling, crisis intervention and safety planning with children, youth and adults demonstrating very “high risk” behaviours, serious emotional/behavioural disorders, and trauma symptoms that cannot be responded to at the community level.
The Case Manager reports to the Outpatient Clinical Supervisor.
QUALIFICATIONS
- Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)
- Minimum level of education required is equivalent to high school graduation and completion of a diploma training program, specific to mental health and social services, at a recognized college
- Registration with a recognized professional body and/or to become registered within a year of commencing employment
- Minimum 1-3 years related experience is required
- Completion of courses specific to cultural competency, trauma, suicide intervention, family violence a plus
- Can function effectively in a fast-paced, multi-tasking, ever-changing crisis environment
- Self-starter with excellent time management skills to effectively maintain a busy schedule
- Can remain calm under pressure
- Problem solving capability
- Counselling, case management, crisis prevention and intervention experience
- Demonstrated experience working with individuals who have experienced trauma
- Experience working with individuals having serious and complex mental health issues is essential
- Knowledge, education or training in working with traumatized individuals
- Must have working knowledge assessing high risk clients and responding appropriately
- Must be knowledgeable and follow duty to report requirements
- Proficient in typing and competent in the use of computers
- Accuracy and strong attention to detail is a must
- Excellent written and verbal communication skills
- Must be able to use an electronic medical record
- Ability to complete documentation in a timely manner
- Extensive knowledge of community resources and/or a willingness to learn
- Knowledge of the people, culture and mental health priorities of First Nations communities in the Sioux Lookout region
- Ability to communicate in one or more of the First Nations dialects of the Sioux Lookout region will be an asset
- Must support and adhere to organizational policies, procedures, philosophy and mission
- Must follow standard clinical operating procedures
- Professional integrity and the ability to maintain confidentiality is essential
- Ability to perform the requirements of the role on a regular basis
DUTIES & RESPONSIBILITIES
- Work with clients and other members of the treatment team to complete needs assessments
- Prepare individualized service plans to reflect client goals
- Provide plan to referral source
- Monitor progress or lack of progress
- Regular check-ins during service delivery to assess if clients are attaining treatment goals
- Work with the clients and team to adjust plans as necessary
- On-going research and to become the expert on all available mental health resources in the region and referral procedures
- On-going rapport building with all partners and potential partners
- Actively explore and connect with First Nation community linkages to establish effective aftercare support
- Discharge planning involving the client, family and treatment team and others as necessary
- Arrange and participate in case conferences as necessary
- Coordination, linking and referring to other resources (e.g. First Nations community-based services, regional services, specialists)
- Work with Nodin’s other services, other SLFNHA departments, and external agencies to ensure clients receive other services if required
- Formalize discharge plans in writing with a discharge summary to include clinical team recommendations and follow-up actions
- Communicate summary of treatment, recommendations and plan to relevant providers to ensure there is integration of services, a smooth transition and continuity of care
- Maintain typewritten client documentation on all direct and indirect contacts according to clinical standards and required timelines
- Actively participate in supervision meetings, department meetings, case discussions, planning for treatment provision, and service development meetings
- Develop and deliver presentations and group work as requested
- Other duties and responsibilities as assigned or needed
