Nodin Mental Health Services is Sioux Lookout First Nations Health Authority’s mental health department. It is a resource available to First Nation children, youth, adults and families from 33 remote northern communities served by SLFNHA. Nodin MHS has several services: intake, counsellors travelling to communities, a crisis response program, community-based children’s mental health and addiction workers, traditional healing, on-call, youth school counsellors, and an outpatient mental health service (OMHS).

The Case Manager is a member of the OMHS multi-disciplinary team. This team provides intensive brief counselling, crisis intervention and safety planning with children, youth and adults demonstrating very “high risk” behaviours, serious emotional/behavioural disorders, and trauma symptoms that cannot be responded to at the community level.

The Case Manager reports to the Outpatient Clinical Supervisor.

QUALIFICATIONS

  1. Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)
  2. Minimum level of education required is equivalent to high school graduation and completion of a diploma training program, specific to mental health and social services, at a recognized college
  3. Registration with a recognized professional body and/or to become registered within a year of commencing employment
  4. Minimum 1-3 years related experience is required
  5. Completion of courses specific to cultural competency, trauma, suicide intervention, family violence a plus
  6. Can function effectively in a fast-paced, multi-tasking, ever-changing crisis environment
  7. Self-starter with excellent time management skills to effectively maintain a busy schedule
  8. Can remain calm under pressure
  9. Problem solving capability
  10. Counselling, case management, crisis prevention and intervention experience
  11. Demonstrated experience working with individuals who have experienced trauma
  12. Experience working with individuals having serious and complex mental health issues is essential
  13. Knowledge, education or training in working with traumatized individuals
  14. Must have working knowledge assessing high risk clients and responding appropriately
  15. Must be knowledgeable and follow duty to report requirements
  16. Proficient in typing and competent in the use of computers
  17. Accuracy and strong attention to detail is a must
  18. Excellent written and verbal communication skills
  19. Must be able to use an electronic medical record
  20. Ability to complete documentation in a timely manner
  21. Extensive knowledge of community resources and/or a willingness to learn
  22. Knowledge of the people, culture and mental health priorities of First Nations communities in the Sioux Lookout region
  23. Ability to communicate in one or more of the First Nations dialects of the Sioux Lookout region will be an asset
  24. Must support and adhere to organizational policies, procedures, philosophy and mission
  25. Must follow standard clinical operating procedures
  26. Professional integrity and the ability to maintain confidentiality is essential
  27. Ability to perform the requirements of the role on a regular basis

DUTIES & RESPONSIBILITIES

  1. Work with clients and other members of the treatment team to complete needs assessments
  2. Prepare individualized service plans to reflect client goals
  3. Provide plan to referral source
  4. Monitor progress or lack of progress
  5. Regular check-ins during service delivery to assess if clients are attaining treatment goals
  6. Work with the clients and team to adjust plans as necessary
  7. On-going research and to become the expert on all available mental health resources in the region and referral procedures
  8. On-going rapport building with all partners and potential partners
  9. Actively explore and connect with First Nation community linkages to establish effective aftercare support
  10. Discharge planning involving the client, family and treatment team and others as necessary
  11. Arrange and participate in case conferences as necessary
  12. Coordination, linking and referring to other resources (e.g. First Nations community-based services, regional services, specialists)
  13. Work with Nodin’s other services, other SLFNHA departments, and external agencies to ensure clients receive other services if required
  14. Formalize discharge plans in writing with a discharge summary to include clinical team recommendations and follow-up actions
  15. Communicate summary of treatment, recommendations and plan to relevant providers to ensure there is integration of services, a smooth transition and continuity of care
  16. Maintain typewritten client documentation on all direct and indirect contacts according to clinical standards and required timelines
  17. Actively participate in supervision meetings, department meetings, case discussions, planning for treatment provision, and service development meetings
  18. Develop and deliver presentations and group work as requested
  19. Other duties and responsibilities as assigned or needed
