Job description
NOW HIRING FOR HAMILTON AND NIAGARA

Are you a passionate and experienced Registered Practical Nurse (RPN) or Registered Nurse (RN), ready to take the next step into a leadership role? As a Care Manager with Stay at Home Nursing, you will have the exciting opportunity to oversee a portfolio of clients, crafting personalised care plans that empower them to achieve their health goals. You'll also play a key role in guiding and supporting caregivers, ensuring they deliver exceptional care that brings positive outcomes and enhances the well-being of each client.

At Stay at Home Nursing, we believe that home is where healing happens. Whether managing chronic conditions, recovering from a medical procedure, or ageing gracefully, home provides the comfort and security that is essential to good health. Our mission is to make comprehensive home healthcare the standard, and we’re looking for motivated nurses who share our passion for transforming how care is delivered at home.

As a Care Manager, you'll be at the heart of this mission, playing a pivotal role in improving the lives of clients and their families. You’ll have the opportunity to see the tangible impact of your work, while also growing as a leader in a fast-growing organisation that values innovation and excellence.

Your role will be centred around ensuring the best health outcomes for your clients and collaborating with our scheduling and HR teams to ensure the perfect caregiver-client match. You will help build a team of caregivers who are as committed to providing high-quality, compassionate care as you are.

At Stay at Home Nursing, we recognize the powerful connection between home and health, and we are passionate about being part of a larger movement to bring comprehensive healthcare to people where they feel most comfortable—at home. Join us in making a lasting impact, one home at a time.
