Are you an experienced registered practical nurse seeking a rewarding career that cares for others, in a professional practice that cares for you? You’re looking in the right place.

As a Care Connector (Status: Temporary, Full-Time Start date: Immediately to October 24, 2025; Location: Any Ontario Health at Home office within the North East), you will help patients navigate through a complex health care system by connecting people to an appropriate primary care practitioner (Unattached Patient Program – UPP).

Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.

What will you do?

  1. Collaborate with patients, their families, caregivers, or substitute decision-makers to find a Primary Care Provider within their community based on their health care needs.
  2. Work closely with individuals to understand their health care needs to make the best match possible.
  3. Provide information and referral assistance to patients to access alternative community resources.
  4. Evaluate patient satisfaction to identify trends and areas of improvement.
  5. Establish and maintain effective relationships with Community Care, local agencies, primary health care professionals, and other appropriate agencies.
  6. Accurately document and chart patient's assessment results, goals, treatment plans, and interventions.
  7. Balance patient needs and choices with available resources, ensuring patient’s values and preferences are respected.

What must you have?

  1. Registered Practical Nurse in good standing with the College of Nurses of Ontario;
  2. Good knowledge of primary care services and other community resources in Ontario Health at Home for the area of North East, as well as the roles of health care professionals;
  3. Relevant experience in a community health care environment;
  4. Practical knowledge and understanding of relevant legislation (e.g., regarding the provision of health care services, privacy, etc.);
  5. Effective planning, organizing, and evaluation skills to manage multiple clients, provide information reports, and take corrective action as required;
  6. Ability to accurately complete required documentation, reports, forms, and recommendations;
  7. Strong listening, interpersonal, facilitation, and communication skills to work with diverse client groups with varying levels of comprehension and language capability;
  8. Ability to establish and maintain a wide range of contacts with health care professionals and community organizations;
  9. Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel, PowerPoint, etc.);
  10. Advanced oral and written proficiency in English is essential.
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

What would give you the edge?

  1. Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics.
  2. Case management experience or recent related community experience.
  3. Ability to speak French or another second language.

French Proficiency: French required
