If you possess a strong foundational knowledge in antimicrobial pharmacotherapy, this role offers the opportunity to deepen your expertise through collaboration with experienced pharmacists and physicians. While prior antimicrobial stewardship experience is not required, exposure to infectious disease management will be valuable as you work to integrate clinical information with the latest literature. You’ll use this knowledge to define optimal patient care while contributing to broader clinical guidance.

About This Team:
The Sinai Health Antimicrobial Stewardship team is an interdisciplinary group committed to optimizing patient care and combating antimicrobial resistance through evidence-based strategies. As a key component of the clinical quality structure at Sinai Health, our program works hospital-wide, collaborating closely with bedside clinicians to enhance antimicrobial therapy practices. We focus on sustainable, system-wide improvements by integrating behaviour change approaches and providing targeted interventions to support optimal prescribing. Our team is actively involved in collecting and analyzing antimicrobial usage (DOT, DDD), cost, and appropriateness data across units at both Mount Sinai Hospital and Hennick Bridgepoint Hospital. Through these efforts, we aim to drive measurable improvements in antimicrobial use and foster long-term partnerships to ensure continued success in stewardship initiatives.

In this role you will:

Core Antimicrobial Stewardship Practice (80%):

  • Lead established interdisciplinary antimicrobial stewardship patient care rounds and provide patient specific feedback to the clinical teams (prospective audit and feedback).
  • Assist in the development and implementation of effective and sustainable interventions that foster improved antimicrobial use, increase patient safety and promote effective resource use.
  • Identify patterns in antimicrobial consumption/resistance and consider possible drivers.
  • Act as a clinical resource on complex cases for health care professionals in a patient-focused practice.

Teaching and Education (10%):

  • Participate in inter-professional teaching to clinicians.
  • Develop and deliver teaching to multidisciplinary/multiprofessional undergraduate and graduate trainees on rotation.
  • Provide preceptorship to pharmacy trainees in undergraduate and graduate rotation.

Knowledge Translation and Quality Improvement (10%):

  • Participate in the development of evidence-based clinical summaries.
  • Collaborate with other hospital stakeholders to identify and participate in quality improvement initiatives.
  • Provide support and mentoring for pharmacy students, residents and fellows QI/research projects in collaboration with the department of pharmacy and antimicrobial stewardship manager.
  • Participate in conferences and regional planning/working groups.
  • Perform other duties consistent with the job classification, as required.

Job Requirements:

Mandatory:

  • Active registration in good standing with the Ontario College of Pharmacists.
  • Completion of a Doctor of Pharmacy (Pharm. D), or recognized equivalent with experience.
  • Advanced clinical designation/degree (e.g. ACPR, second pharmacy degree).
  • Three to five years of relevant hospital-based pharmacy practice experience.

Preferred:

  • Antimicrobial stewardship/infectious disease experience.
  • Teaching experience.

Skills and Knowledge:

  • Interpersonal skills.

  • Written and verbal communication.
  • Time management and organization.
  • Basic knowledge of quality improvement principles and practices.
