This position is open to all qualified OPSEU staff throughout Lakeridge Health commencing April 8, 2025 and closing at 11:59 pm on April 15, 2025.

Duties And Responsibilities

The Social Worker will report to the Patient Care Manager, Healthy Aging Program Social Work and work collaboratively with inter-professional team members and community partners. Responsibilities include the provision of social work interventions/services such as psychosocial assessments, discharge planning, crisis/trauma/conflict management, individual/group counselling, education, facilitating case conferences, data collection, and assisting patients and their families in adjusting to their health care needs. Key components of the role include supporting the utilization of a home first philosophy while working with the patient/family and the inter-professional team on complex patient transitions utilizing short term solution-focused interventions. The incumbent will cover multiple areas/programs requiring social work interventions on various patient care units/committees across all Lakeridge Health sites as assigned.

Qualifications

• Master of Social Work Degree; registration, in good standing, with the Ontario College of Social Workers and Social Service Workers;
• Experience with inpatient mental health and substance use populations, and provision of care through a range of modalities;
• Demonstrated skill in mental health assessment, crisis management, and knowledge of applicable legislation;
• Demonstrated experience in working collaboratively with inter-professional teams and community support agencies;
• Experience in delivering comprehensive social work interventions within Medicine, Surgery, CCC, Critical Care, Rehabilitation, and Women's & Children's/ER;
• Experience providing comprehensive transition management/discharge planning while utilizing knowledge of levels of care, applicable legislation, and community resources;
• Demonstrated ability to organize workload and establish priorities effectively within a fast-paced environment with a varied caseload;
• Excellent communication skills, both oral and written;
• Demonstrated effective leadership and interpersonal skills;
• Experience with program development and evaluation;

• Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient, and environmental safety.

Conditions Of Employment

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.
