Position Overview:

Under the direction of the Early Years Program Manager, the Access Case Manager is responsible for the continuous implementation of the case management system. This includes ensuring regular, appropriate documentation on all clinical files; screening and developmental assessments for waitlist clients; regular conferencing/reviews of relevant cases; and evaluating system effectiveness. A primary responsibility is to ensure the provision of case management and early intervention services for the 0-6 population in collaboration with the access team that is consistent with the philosophy, policies, and standards established by Dilico Anishinabek Family Care. The access case manager will support with internal and external referrals to better support families coordinate services in a timely manner.

Qualifications and Requirements:

  • A degree or related diploma in social services will be considered if complimented with clinical experience in children’s mental health focusing on ages 0-6
  • Experience working with Anishinabek children, families and communities
  • Minimum three (3) years mental health case management experience with clients
  • Minimum three (3) years experience providing individual, family and group counselling, preferably with Anishinabek clients
  • Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered
  • Experience in developmental assessments and screening tools
  • Satisfactory Criminal Records Search; including a Vulnerable Sector Check
  • Ability to work flexible hours as determined by the Early Years Manager in relation to service requirements
  • Must posses a Class “G” Driver’s License and have access to a personal vehicle
  • A valid Standard First Aid/CPR Certificate is recommended
  • Must complete and maintain certification in CANS and other relevant Ministry of Children and Youth Services Initiatives
  • Ability to travel regularly to District communities

Hours of Work: 32.5 hours per week

Salary Range: $29.26 To $38.18 Hourly
