Responsibilities:

1. Conduct assessments and create care plans for patients in their homes.
2. Administer medications and perform treatments as prescribed by the patient’s physician.
3. Monitor and document patient vital signs, symptoms, and reactions to medications or treatments.
4. Educate patients and their families on their medical conditions, medications, and treatments.
5. Coordinate care with other healthcare providers, including physicians, therapists, and social workers.
6. Communicate regularly with the patient’s physician to ensure continuity of care.
7. Maintain accurate patient records and documentation.
8. Provide emotional support to patients and their families.
9. Provide medical support to nursing staff, ensuring coordinated patient care and accurate administration of treatments.
