JOB DESCRIPTION
Responsible for providing administrative support to the Health Information Management Department along with its sections, specifically by providing support to the HIM Officer in day to day operations pertaining to a variety of Health Information Management functions, such as Filing and Retrieval, Release of Information, Chart Analysis. Also responsible for providing excellent customer services, processing actions, performing day to day tasks in the Health Information Management Department. Performs variety of clerical duties in maintaining the patient medical records/ Health Information that include arranging the contents of records, filing, retrieving, distributing and data entry of requests receiving and releasing in the HIS.
RESPONSIBILITIES
- Receives and files inpatient discharged medical records
- Cross-checks records with dispatch logbook. Applies established guidelines and policy governing the release of patient information
- Processes the day to day operations within the filing and retrieval section
- Assists in any other relevant task that may be assigned
- Secures the filing room from unauthorized access
- Discusses with respective officers before recommending solutions
- Extracts data from Cerner and tabulates it onto relevant templates
- Performs first level accuracy and quality checks on the data collected and contacting the data resources in cases of inconsistencies
- Preparing the simpler daily, monthly and annual reports
- Maintaining and Promoting Health Information Management
- Works cooperatively with other Clinical & Non-Clinical staff to ensure data integrity and validity.
- Utilizes all computerized applications
- Orders and restocks office supplies
- Photocopies documents as needed
- Record keeping for essential day-to-day work
- Reports any Information Technology related problems to IT support desk and following-up for resolutions
- Assumes of immediate Health Information Management officer in his/her absence.
- Maintains the filing system in effective way to prevent wrong filing or missing records
- Maintains the medical records tidily by changing torn or damaged folders
- Assembles the contents of medical records in the prescribed order for easy retrieval of information
- Creates volumes of larger records and maintain the data in the HIS
- Purges inactive records and maintains the details of inactive records in the HIS
- Receives medical records returned from clinics, departments, inpatient discharges and tracks in the HIS
- Generates daily list of records not received in HIS and follows up to receive the pending records
- Receives and files external lab reports, referral letters, insurance authorization forms and other correspondences in patients’ medical record
- Provides medical records and information on patients for clinic appointments, inpatient admissions, Emergencies, clinical review, research and Audits
- Provides photocopies paper medical records/ prints copies of electronic medical records as directed by the ROI supervisor
- Ensures that an active Encounter is available for each PIR before processing the same.
- Ensures that loose sheet received from various clinics and nursing station are properly sorted and filed in corresponding Medical Record based on MRN
- Ensures that entire medical record movement from and to HIM is properly tracked through HIM Chart Tracking Module using Tracking ID
- Runs the record reconciliation report on daily basis and follows up with records not returned
- Informs section lead and Senior HIM Officer regarding any missing records not returned to HIM
- Maintains confidentiality with regards to any information exchanged or received in the current capacity of role in accordance with facility policy
- Sorts, scans, uploads, checks quality and validates clinical documents (External & Internal) received
- Ensures patient confidentiality while dealing with patient health information
QUALIFICATIONS
Experience :-
Required:
- 1-2 years of relevant experience
Desired:- Experience in a large healthcare facility
Educational Qualification: Required:- Diploma in Health Information Management or equivalent or in relevant field
Desired:- Bachelor’s Degree in Health Information Management or equivalent
