Job description

ROLE AND RESPONSIBILITIES:

Knowledge & Reporting

  1. Has knowledge of all changes of the patient condition and takes proactive action for their care.
  2. Is able to design patient care / management plans according to the needs of the patient.
  3. Interacting with the health care teams for maintaining harmonious relationships.
  4. Has Knowledge of Healthcare and medical terminology.
  5. Evaluates medical supervisor based on the procedures and documentation.
  6. Aware of departmental Policies, management plan of care & forms to be documented.
  7. Knows and complies with departmental communication channels.
  8. Knowledge of PAMS system usage.
  9. Knowledge of Incident Reporting.
  10. Aware of Patient rights and responsibilities.

Patient Supervision & Assessment

  1. Does the initial assessment of patient and the follow up on the patient condition in a timely manner.
  2. Meets and discusses patient health & vitals with the medical supervisor on a daily basis.
  3. Checking all relevant documentation on a daily basis.
  4. Making sure the availability of medical supervisor and caregivers on a daily basis at the patient residence.
  5. Checking on whether all management protocols are being followed at the patient house doctor is responsible.
  6. Acts as an intermediary between the treating physician, government agencies, health care worker of the company & management.
  7. Checking vital signs (temperature, blood pressure, pulse and respiration) during visit.
  8. Updating medication in the software.
  9. Wound checking / progress.
  10. Assessment of physical, mental and neurological status.
  11. Check the cleanliness of the patient and patient’s room.
  12. Assessment of peg tube for function (intact), checking the stoma for granulation, exudates and redness.
  13. Doing skin, pain assessment on a timely manner.
  14. Provide appropriate patient data to the patient/ family.
  15. Evaluates the medical supervisor on their performance according to the company protocol.
  16. Implementing the medical forms.

Patient Care

  1. Visits the patient every 2 weeks and reports his/her condition effectively to the Department Head.
  2. Knowing and Implementing the management plan for all the patients.
  3. Making effective care plan in a weekly basis.
  4. Ensures complete written physician’s order.
  5. Adheres to the procedural procedures; skin, pain assessment.
  6. Maintaining hygienic and safe working environment in compliance with the health care procedures.
  7. Correctly identifies patients by using a minimum of two forms of identification before starting any procedure.
  8. Completing appropriate forms.
  9. Communicates effectively with patients / family.
  10. Maintains BLS / CPR skills.
  11. Applies infection control techniques, (e.g. Hand hygiene, Using PPE, Medical waste management etc..).
  12. Knows Fire Plan & evacuation procedure.
  13. Has knowledge of Environmental safety awareness.
  14. Has knowledge of implemented departmental quality assurance programs / KPIs.

Patient Interaction Competencies

  1. Listening to patient & talking to patient.
  2. Knows patient’s history – medical and social.
  3. Ensure patient privacy & confidentiality during assessment.
  4. Provide patient & family education.
  5. Alerts patient on patient rights & responsibilities.

Documentation

  1. Provide care plan to the medical supervisor to each patient.
  2. EMR of the patient to be filled and checked.
  3. Care plan to be documented.
  4. All relevant forms to be completed.
  5. Medication and Injection Administration.

QUALIFICATIONS

  1. Medical Degree or qualified as a General Practitioner.
  2. Must have 2 years of experience.
  3. MOH license and board certification in family medicine or general practice.
  4. Strong diagnostic and treatment skills for a wide range of medical conditions.
  5. Excellent communication and interpersonal skills to effectively communicate with patients, families, and other healthcare professionals.
  6. Ability to work collaboratively in a team-based environment.
  7. Strong organizational and time management skills to efficiently manage patient care and documentation.
  8. Commitment to providing high-quality patient care and continuous professional development.
