Job description
1. Home-Based Diagnostic Screenings & Preventive Nursing Care
- Conduct comprehensive diagnostic screenings at patients' homes, including:
- Blood sample collection for laboratory analysis.
- Metabolic and functional health assessments.
- Assess patient health history, lifestyle, and risk factors for early disease detection.
- Collaborate with physicians to implement customized wellness and preventive care plans.
2. Functional & Regenerative Medicine Support in Home Care
- Assist in administering and monitoring functional and regenerative therapies, including:
- Home-based IV therapy (customized nutrient infusions, NAD+ therapy, ozone therapy, vitamin infusions).
- IM injections for peptide and exosome therapy.
- PRP therapy preparation and regenerative wound care management.
- Pain management through holistic and alternative therapies.
- Educate patients and families on integrative health strategies aligned with functional and regenerative medicine.
3. Chronic Disease Management & Holistic Home Nursing Care
- Provide comprehensive nursing care for patients with chronic diseases, including:
- Diabetes & metabolic disorders (home glucose monitoring, lifestyle coaching).
- Cardiovascular conditions (hypertension management, ECG monitoring).
- Autoimmune & inflammatory diseases (pain management, nutritional support).
- Post-operative recovery & wound care (monitoring and rehabilitation support).
- Monitor patient progress and update electronic medical records (EMR) for home-based health tracking.
4. Patient Education, Family Training & Lifestyle Coaching
- Educate patients and families on preventive health strategies, disease management, and medication adherence.
- Provide nutritional counseling, hydration therapy, stress management techniques, and sleep hygiene guidance.
- Train caregivers and family members in basic health monitoring, emergency response, and safe medication administration.
- Offer remote health guidance and telemedicine support when needed.
5. Compliance, Medical Ethics & Clinical Standards in Home Care
- Ensure full compliance with DHA regulations, home care safety protocols, and infection control measures.
- Maintain accurate and confidential patient records to document progress, symptoms, and treatment responses.
- Adhere to sterile techniques, safe medication handling, and patient safety procedures in home care settings.
- Stay updated on continuing medical education (CME) requirements and best practices in home-based functional medicine nursing.
6. Collaborative & Multidisciplinary Approach in Home Care
- Work closely with physicians, dietitians, regenerative medicine specialists, and wellness practitioners to ensure holistic care.
- Coordinate home visits with lab technicians, physiotherapists, and rehabilitation specialists as needed.
- Participate in virtual case discussions and medical team meetings to optimize patient care.
- Engage in training programs for home care nurses to enhance professional development and skills in functional medicine.
